Hi this is my first time on this form, wanted to get feed back on a problem. I an type 2 and have been for a good 17 yrs. My Dr. Has me on 1500 mg of metformin daily (500 3x a day). Also 28 ml of Lantus twice a day. I am to take a metformin 30 min before ea meal and the dose of Lantus morning and night. I normally take the last dose of Lantus right before bed (different times). I have been waking up with hypo in the middle of the night or very early morning. My last A1C report was 6.0 it had come down from 9.5, but the Dr. Still has me taking a lot of med. Is this really necessary? I feel like, now that I am at a pre-diabetic range with my A1C that maybe I don't need so much and maybe this is why I am having hypo attacks so often?

Hi and welcome,

If you are having a lot of hypos, then yes, it sounds to me like your insulin dose needs adjusting downwards. You shouldn't be having to suffer this and it must be very scary for you. However, I don't and never have taken insulin, so I am no expert and not a doctor.

What sort of levels are you seeing before bed (before you take the Lantus) and how low are you dropping, and how many units of Lantus do you take?

First I am type 2 not type 1, I think there was a typo.
My levels after dinner usually run between 150 and 179. I take 28 no of Lantus at bedtime and in the middle of the night / early morning has gone as low as 59- 70. The other night it had gone to 63, I awoke, sweating, shaking and felt like I needed to vomit. I took a couple glucose tablets which helped get it up again. I had told my Dr about this and the 28ml twice a day of Lantus is actually less then what I was taking before. She previously had me on 32 no.

Hi Cyndie,

As I said, I'm not expert enough about insulin to help you. I urge you to go back to the doctor and tell him about these night time hypos and how low you are going.

I see you are a type 2 on insulin despite it says you are a type 1 in big letters at the top. Type 2's on insulin are in a bit of a minority around here which might explain the lack of answers. Maybe a type 1 could help if there are any about.

Cyndie - Would you mind if I changed your thread title to take away the reference to Type 1? I feel it would stop any confusion and might even be inhibiting appropriately knowledgeable members making comment.

As @Bluetit1802 has suggested, it certainly sounds like something probably needs tweaking.

Are you confident in treating your hypos? If not, then a read of the following page might be helpful: https://www.diabetes.co.uk/how-to/treat-a-hypo.html

The site also has a Hypo Programme:

Hypo Program - improve your understanding of hypos. There's a version for people with diabetes, parents/guardians of children with type 1, children with type 1 diabetes, teachers and Health Care Professionals.

There's also a link in my signature.

Let me know about your thread title, please. It only takes a moment to modify.

@Cyndie, as a Type 1, when I was on Lantus, I was told to take it at the same time every day. I firstly took it at 10pm (give or take 30 mins) and started going hypo early morning. My DSN suggested taking it in the evening, so I started taking Lantus around 6pm, just before my evening meal. The hypos stopped.

You said your evening dose is before bed (different times). You could try taking your second dose at the same time every evening, rather than varying it. If that doesn't work, maybe a reduction in dose is required. If your doctor hasn't told you about reducing your insulin, check with him/her first.
