[QUOTE="Hopeful34, post: 2521586, member: 378858"] Post offices have the D1 application forms, but not the DIAB1 form, and neither did my gp surgery. There is a delay in sending out renewal letters, or there was at the end of last year. Could you ask a friend to print out the DIAB1 form, or do it at your library? Just to let you know though, that I did that, and the online DIAB1 form is out of date, and they sent me a more up to date one 6 months after receiving my application? When you fill in the form, I'd write never because I've just moved house, under the GP box, and give them the details of your hospital consultant. Re who should they contact, it's up to you if you fill in one or both. Personally I'd always give my endo's name and details as they're the specialist, and as you've not seen your GP, you'd need to ask your hospital team for a covering letter to continue to drive if your licence runs out, as In Response said.
