I don't think there are any that really do blood glucose measurement - there may be some in the development pipeline, but nothing that really works just yet.
The closest is that the Dexcom CGM will be (is?) able to directly communicate with an Apple watch so that you get a reading without needing to look at a phone, but that's the closest we are at.
What do you mean by "exact recipes" by the way ?
Many of us here (and me included) find that swapping out sugars, starch and seed oils for fats is a sustainable way to keep enjoying food, and get insulin and BG under control.
Are you planning on joining the NHS meal replacement program, is that what you mean by the 12 week plan?
