What am i doing wrong

need some advice currently taking 21 units of Lantus at night levels seem to be ok between 4.5 - 8 through the day might be a bit higher before bed as i tend to have a large tea but do try and keep carbs levels down which seems to work on my levls however i am losing weight i was 21 stone now down to 15 1/2 i am 6'6 so look like a rake if i bring this up to my dn all i get is your bmi is great but i feel so tired all time and ache in my joints

how long did it take you to lose the weight?

If this was rapid weight loss then it's bound to have a negative effect as you should to this gradually.

You DN is likely to think diabetes first, why not visit your GP and ask for a simple blood test as it could be something simple like anaemia.

As you may have dieted to lose the weight you may just need a boost of extra vitamins and other supplements to boost your energy levels.
 
Take a look at your insulin/carb ratio at tea-time as this could be wrong and explain your higher readings before bed.

As for losing weight, if it's unintentional and you haven't increased your physical activity or reduced your food intake then I suggest you see your gp and discuss, not everything is down to diabetes and you really need to have checks done to rule out any other cause.

3 stone i lost in 3 weeks just before diagnosis that was 6 months ago llost another 2 since then i suppose i may be being paranoid as i dont drink anymore and have significantly reduced carb intakelast Hba1c was i think about 6 what is carb /insulin ratio ? i just have 1 dose of lantus at about 10 at night ?
 
Maybe your Lantus is running out in the evenings.

I had this and was on around 36U of lantus and was actually compensating with higher ratios for Rapid at lunch and even higher at dinner.

I was swapped onto a split Dose of Levemir and that sorted everthing out.

I also found that I didnt feel as lethargic and also I generally felt a lot better in myself. I know plenty of people have issues with Lantus and I didn't think I did until I came off it.

I would suggest you speak to your DSN and show them your diary to see if they agree that maybe a split dose would work better for you.

I agree with Andy about trying for a split dose. Definitely speak to your DSN and tell her how you feel and ask her to take a look at your diary.

Insulin to carb ratio is where you match your insulin to the carbohydrate in your food to normalise your bg thereafter, for example I need one unit of insulin in a morning to match 10g of carbs but this increases to 1.5 units on a lunch time to match the same 10g of carbs, your requirement may be less or more, as we are all individual we all have different insulin requirements.

The DAFNE course (dose adjustment for normal eating) is a very worthwhile course to take which teaches type 1 patients the principles of carbohydrate counting and matching their insulin to suit, it also cover loads of other things relevant to diabetes such as what to do when you are sick etc, ask your diabetes nurse if DAFNE is available in your area.
