What do the results of my c-peptide results mean?

Hey all,

I was diagnosed with diabetes about 4 months ago (blood test conducted due to rapid weight loss over the prior 2 months). A1c of 95, I was basically DKA upon admission to hospital.

I'm 30, bmi of 20, no family history. Initially they thought I was type 1. I had a GAD test and the results were normal so the nurse thought i would be type 2.

I just received the results of my c-peptide test that was organised through my GP, but he doesnt actually have much knowledge of this test so i'd like to ask you guys!

- insulin 12pmol/L (normal range 18-173)
- c-peptide 129 pmol (normal range 370-1470)

Obviously these results are below the normal range, but where does this place me in the type 2 vs 1 spectrum given that my GAD results were normal?

Also, my a1c result after 3 months was 5.4 and ive found it generally okay to manage my blood sugar using insulin (i kind of assumed my diabetes was 'mild' or that my body was still producing insulin).

i think your GP should refer you to a endocrinologist. from there you will get all the help you need to determine what
type or if you even have diabetes.

I was referred to an endo 4 months ago and the appointment keeps getting rescheduled. It's apparently going to be February.
that hasn't been my experience - i saw my endo in 4 months ago and i am due to see him in January.
surely the endo would have got those results or he may have requested them. and he may very well have checked those results but will tell you in February? i hope you do see the endo in February

The anti GAD is not the only antibody, it might be you have another one.

Having diabetes plus a positive antibody result is diagnostic for T1.
But testing negative is not diagnostic for T2.
 
It’s perfectly possible to be type 1 with a negative GAD result. Maybe you need to take the nurse with you to the endo when you finally get the appointment so she get educated and give more accurate information in future. Your tests results look to me to be noticeably below “normal” which sounds exactly what would be expected in type 1. Type 2 tend to have high levels not low. In your shoes I’d see no reason to question the existing diagnosis and ask the endo when you eventually get there.
