When I first went on the premixed insulin I asked my pharmacy what do I have with the first injection as I don't eat til 5. Hence missing breakfast that I guess your supposed to have in our society ( with the first injection). He said I could have a glass of milk. So since me or my family don't like or buy milk what can I have. And when I'm substituting for milk what is the main quality or ingredient substitute. The carb content or fat or protein.?
A liquid.
 
This is something you would need to return to the phsrmacy with @cdpm .
If they as health care professionals have given advice cannot, for some reason, follow it would have been best for you to have told them of your aversion to drinking milk.
Our members cannot give advice on how to take your medication.
 
likely the carb content, though I'm somewhat confused as even drinking a gallon of milk at breakfast time isn't going to help you last until 5pm.

What type of premix are you using, what dosages, etc.?
