on linagliptan and metformi for last few years, bg have started to go high in last few months, hba1c was done recently but comes back normal. I’ve seen a few readings above 18mmol, average over last week has been 9.2mmol. GP unwilling to do anything as hba1c is normal, though despite giving me pills for last 3 years they had me down as resolved in 2018! They had taken me off linagliptan last year because hba1c was normal, but I then started to get 25mmol readings So back on it
I’m concerned about the peaks, but should I be? I got one of those libre meters and I’m over 10mmols 33% of the time. I am feeling ill most of the day unless I eat no food, up until I get so hungry I feel sick. So it’s a bit of an annoying cycle.
Is it worth pushing the GP for something else? Should I just accept these highs, as they’re not as high as I’ve previously had?

oh and I’ve just read that linagliptan can give you blurry eyes, I’ve been having that the last few years, thought my eyesight was going!

I'm going to start by stating that I know absolutely nothing about Linagliptin, although having just looked it up, it appears to promote the production of insulin and is usually prescribed when healthy diet and exercise alone do not have the desired outcome.

High blood glucose levels can also mess with your eyesight because your eye lens swells but this often returns to normal when a person gets blood glucose under control.

As for the peaks, I would be concerned in your position. Every time you go that high it is doing a tiny bit of damage to your body, mainly your eyes. What we cannot see here is the size and duration of the peak. We try our best (not always successfully) to keep the rise in glucose levels from pre meal to 2hrs after eating within 2mmol/L. So I'd suggest evaluating this as a next step.

I don't want to appear rude but what's your diet like? Are you consuming a lot of carbohydrates ?
 
You are right to be concerned about your BG peaks, especially as you feel ill most of the day unless you eat no food.

Do you have the two most recent HbA1c results to hand? If not and your HbA1c numbers are not available online, I'd give your GP practice a bell. A receptionist should be able to read the results to you over the phone.

Do you have anemia or another red blood cell disorder that can skew the hba1c that you know of?
In people with a condition causing the hba1c to be unreliable, often a fructosamine test is used instead so you might want to ask for that.

Do you know the exact number of your hba1c? 'Normal' doesn't tell you much.

An average of 9.2 should give you an hba1c of around 57.
Those very high readings, were they caused by particularly high carb foods? Have you tried any adjustments to your meals to avoid those very high spikes?

Do you get those high numbers when you eat low carb foods as well? Most of us find we can eat things like chicken legs, bacon and eggs, cheese etc without going very high, which could be very helpful to save you from going hungry.
