Get yourself down to occy health. I'm also a Midwife, with T2, working under the same conditions as you. T2 is covered by the disability discrimination act, use that bit of knowledge to your advantage. You are entitled to ask for a phased return and they can help you put a plan in place to support you in your early days of returning to work.
Ask for coping with the condition doing shifts, it's actually not that bad. The key thing here is be well prepared, well in advance.
