I'm wondering if anyone can offer any suggestions as to which insulin I'd need to adjust.
I currently inject 20 units of Lantus at 8am each morning. I eat a moderate amount of carbs with each meal (always 30g). I've been using the ration of 1 Unit for 15g of carbohydrate for the last 9 years. I've recently been testing on a regular basis and although my pre-meal levels are always between 4 - 8, I always reach between 10 - 14 two hours after eating. My BG is always around 8 when I go to bed and 5-6 in the morning. My BG is always back within normal range after 4 hours of eating, but I'm wondering if I need to slightly up the basal amount or if I should just inject say 30 mins before eating and then eat. Any ideas on how to avoid these BG spikes?
I would suggest injecting 15 mins before eating, 30 if u can get away with it. Test every 15 mins while trying. This way you should catch the spike of the food. As your levels are back in range after 4 hours , suggests the insulin amount is correct, just not the timing yet.
Thanks for your reply - I was thinking that it might be more of a timing issue as opposed to a dosage issue. I guess it's just going to be a matter of trial and error until I get it right

Hmm. I'd never thought about that to be honest. It might be worth trying the timings for a week and adding protein/good fat for a week and seeing if the results are different. I really should know all this but I suppose it's better late than never!

I would have thought the Bolus might need increasing a bit as the ratio of 1 unit to 15gm is a bit lower than many have? The Basal sounds about right as the morning fasting is as low as you would want to go
Daibell - thanks for your reply. I have a 'junior' pen that allows half doses so I was thinking of perhaps upping my Bolus by a half dose each time as I always hypo when injecting on a 1 unit for 10g ratio. Several suggestions to try so hopefully one will work!
