Type 2 on 1000mg slow release and low carb

Good Day,

I have high fasting insulin levels 20.7 m/IU/L, over the past 15 years my fasting insulin has been increasingly high and I gained 13kilos, I started with a low carb diet in 2013 and the doctor put me on 500mg Metformin twice a day, I managed to get the weight off with low carb and exercise, however as soon as my fasting insulin levels came down to normal the doctor took me off Metformin, I continued with low carb and exercise but within a year despite my diet and intermitant fasting I gained back all of the weight, nothing I can do seems to be helping, I returned to a endocrinologist in December 2020 and she put me on 1000mg Metformin slow release once a day, I have been taking it for 6 weeks now and continuing with the low carb diet - 30g, I only lost 1 kilo and my weight is refusing to budge, I have a small frame and 1.59 in height so the excess weight is causing my knees to ache badly, kindly advise as to where I am going wrong ? I will return to the doctor in March but I'm completely baffled as to why I cannot get my weight down. I only eat two meals a day usually around 10 and 6pm and lots of water in between.
 
That certainly sounds odd, and sorry you are having challenges

Do you mind me asking your age?
It’s a very common issue when we get to peri menopause or menopause that we cannot shift the weight,
I’m exactly the same,
The only way it goes is if I completely stop eating.

I'm 45, the doctor checked for menopause and everything seems fine however she did say that she will do more extensive tests the next time, I don't mind not eating lol, whatever works I'm willing to try it, this insulin resistance is the story of my life, I feel so unfit and uncomfortable, I will not stop until I get results.

Personally I found that reducing my eating window even further was fairly beneficial.
I tend to have a light snack at about 3 pm and then dinner at 7 pm wonder if that might help.

Breakfast 10.30am - Usually my most frequent breakfast -Two eggs fried in clarified butter with bacon, green peppers, little onion and mushrooms or any other low carb veg.
or
1/2 Cup Unsweetened Double Cream Yoghurt with low carb granola (Pumpkin seeds, Flaxseeds, Pecan Nuts, coconut Flakes) and a boiled egg with some lettuce or cucumber and cream cheese or
low carb porridge with butter and cream with 10 Walnut halves.
or a green salad with olives, cucumbers, eggs, tomatoes, lettuce, avocado, I make a dressing of avocado oil/olive oil and lemon juice and double cream yoghurt.

I have a cup of coffee at 6am with 2 teaspoons cream and 3 drops of Stevia

Dinner is usually medium protein and low carb veg all prepared in butter or olive oil or coconut oil, usually Chicken thighs or Hake/kingklip fish or grilled Steak with fat.
Twice a week I will have 2 small slices of low carb bread or a low carb wrap with my dinner.
I am not hungry after eating and if I do feel like a snack I usually have about 10 Almonds or Walnuts or a boiled egg or Salami
Like I mentioned I have been doing low carb since 2013 but the past 2 years have been a nightmare, no amount of high intensity gyming, fasting or eating right seems to be working - I am also on Ferrimed Iron Tablets since December 20, I have been taking iron meds now and then as prescribed by the doctor when iron goes low.
She also recommended that I take Alpha Lipoic, Omega 3, Vitamin C and Vitamin D 5000iu as it was very low.
I drink around 2,5 litres of water a day.

Thank you, I will try doing this and see - I was doing the 18/6 intermittent fasting for 3 weeks and nothing.
