Hi all!

I’m moving for the first time whilst also moving to a different area and was wondering if anyone could advise what I do in regard to my diabetes care.

What is the process like to register with a new GP and have prescriptions sorted? I’m worried about losing access to sensors and medication etc!

Will my GP also contact my diabetes team at my current hospital or do I do this independently? It would be too far to travel for appointments but the closest hospital to me is a different trust.

The GP bit is easy. When you've moved, pop into doctors, fill in forms and it should work from there. There's usually a form to fill in to say your ongoing conditions and whether you're under the doctor or hospital for it.
I ended up filling in two sets for two different drs recently as the first one announced closure within a week of my filling it in!
I would probably ask for an extra early prescription so you know you're covered in the interim.
I would also ring the drs you think you're going to register with and talk to them about how long things take. They are usually really helpful.
 
I seem to remember the hospital consultant did the referral for me to the new hospital when I moved area (~25+ years ago - so it may have all have changed, but I'd ask them I think rather than risk being pulled out of the hospital outpatient system and put solely under the care of the GPs - they don't see as many T1s so don't have the experience IMO)

What normally happens is that you register with your new gp, all your previous records will transfer to your new gp, then he/she will refer you to a new consultant. It's not usually done from consultant to consultant, referrals go via the GP, at least it does in my area.

We are moving the week after next.

I’m ( only) pre diabetic not really an issue for me but my wife has a nasty complaint.

We have joined a new GP already and they’ve been great. My wife already has an appointment to see GP.

Unbelievably my wife and I have ordered prescriptions for the new place . The new GP is so far offering great service, it’s a weight off our minds.

All I’m saying is you might be as well to register and be in the system.

Just to say @Brookemurph as a type 1 you are eligible for a sensor wherever you are in the country. Make sure all your medication is up to date and well stocked before you move. This will give you time to register with a new GP. When I moved 9 years ago it was all straight forward and quick. In fact my new Gp is far more efficient.
