Levemir to Tresima

As Levemir is being discontinued, I have been prescribed Tresima as my new basal insulin. At the moment I have 12 units of Levemir twice daily. Does anyone know what my new dose of Tresima should be (I think I am supposed to inject just once every 24 hours)? I read somewhere that when making this change you should roughly reduce the dose by 20% so I’d be looking at 19.2 units. Can anyone confirm if this is correct?
Thank you!

You are best to ask at your surgery either your Dr or DN. we cannot advise this kind of stuff on here

Just to confirm it is against forum rules to advise on medication doses. If they have changed it without giving any information then I would definitely contact your DSN.

When I changed from a different long acting to Tresiba a couple of years ago, my diabetic nurse and my pharmacist told me to keep using the same dose.
I wanted to see what happened first, so I reduced the dose by some 15% to start with, and I was back to my old dose within a week.

Hi @Bwobwob
Welcome to the forum.
I am also changing from Levemir to tresiba but not until after easter.
I have been advised by my GPs surgery Pharmacist (i didn't know the surgery had its own ) who was about 12 to reduce my dose by 20%.
I pointed out i intend to go lower and explained at length the benefits of my libre and its alarm and how 4mm needles had contributed to my hba1c of 39 or 5.7.
I'm pretty sure i bored him to death and he just agreed with everything i said!

There are other posts on here about people changing from levemir which are worth looking at.

I wish you well

Tony
