Hi, I have osteoporosis of the knees and experiencing pain can anyone recommend the best painkiller that isn’t going to affect liver and kidney function as I’ve heard some can cause issues for diabetics I’m type 2,14 years.
 
Presumably you mean osteoarthritis of the knee? You can safely take paracetamol at the stated dosage. Long term you can alleviate your knee pain by improving the strength of the surrounding muscles/tendons/ligaments and reducing the load on the knee joint.

Exercise your legs through squats, lunges, step ups, leg extensions (not working with any undue pain). Losing weight will ease the load on the joint and hence the pain.

The end game for unrelenting arthritic pain would be a knee replacement.

I have a very dodgy knee & can’t take any NSAIDs as they are as you say processed by the liver. I spoke to the pharmacist who said I could use the neurofen gel directly onto my knee as it’s processed differently, I also spoke recently to my consultant as I have a liver condition not connected to diabetes & he said it was fine to use the gel when needed, but please double check with your own pharmacist first

I have osteoarthritis, it’s in all my joints. I have had several operations on my knees due to the osteoarthritis. It’s hard, strengthening muscles to support your joints brings about pain and swelling, but if you don’t the muscles around your knees become weakened creating more strain on your knees. It’s a vicious circle. Ultimately, exercise does help.
As for painkillers I use extra strength Tylenol ( Paracetamol) . I also use Robaxacet extra strength. It helps me (I think you can get it in the UK) It contains Acetaminophen and Methocarbamol. Like @lovinglife I can’t take NSAIDs as they give me stomach ulcers.
 
Maybe also try a magnesium spray, I use it and a lot of my elderly clients do. For me it works but weight management and exercise work also. I dont have the "crunch" and pain now so perhaps that's helped. I also take a biotin collagen supplement and no idea if its working, I only started taking it as I dont eat beef and noticed my hair, nails and skin were not looking great.
