It might be beneficial if you discussed with someone about how you can reduce carbs in your diet. The nurse doesn't appear very helpful imo.

I've been on 40mg gliclazide for just under 29. years and have had only one hypo (which was horrible but not life threatening). The hypo was caused by too much alcohol. It might depend on how much mg of Gliclazide you're taking.
The complacency thing is deadly. It creeps up on you and I've had the strange experience of watching my bgs climb transfixed like a deer caught in the headlights of a car - transfixed as in taking no action to reduce the bgs!
You're doing really well!. I don't know how long since you've been dx'd but the sooner you get those figures down to best levels the easier it will be. I was dx'd in 1997 and really didn't take your approach until a few years ago (with a few good periods in between) but now I'm getting results...
How are the practices being rewarded for achieving 40%+ compliance to bp targets? Isn't there a risk that patients will be pressured because the practices want to receive those rewards?
I never saw the Diabetes anti stigma campaign. Maybe one of the main causes of stigma is that people get confused with the alleged link between being overweight and diabetes. It would be interesting to see what Diabetes UK's stance is on this.

Maybe food companies are not required to put the GI on nutritional labels as we could end up being overwhelmed by too much information? After all, why stop at GI - why not also show the Glycemic Load which can be helpful? And then, and then, where would it all end?
