Long-term insulin user (27F) — lipohypertrophy + abdominal changes. Looking for shared experiences or management options.

I’m 27, Type 1 (diagnosed at age 3), and have been on insulin for most of my life. I have diagnosed lipohypertrophy in my abdomen, upper arms, and thighs.

More recently, I’ve noticed persistent abdominal protrusion, tissue thickening, and an uneven/distorted surface, even when I completely avoid injecting in that area (I have attached an image - it is not of me but a good example of what my stomach looks like).

I’m raising this with haematology/endocrinology, but I wanted to ask the community:

– Has anyone experienced abdominal changes beyond lipohypertrophy?
By this I mean thickened tissue, firmness, and changes to shape/contour rather than just small injection-site lumps.

– Did it affect insulin absorption or comfort?
I often run high despite correct (or increased) dosing, and absorption feels unpredictable. I struggle to trust injection sites.

– Were any management options helpful (medical, referral, or otherwise)?

This has become significant enough to affect daily comfort, clothing fit, confidence, and quality of life. I’m not looking for weight loss advice — just shared experiences, insight, or practical guidance from others who’ve dealt with something similar.
Any thoughtful responses would be really appreciated.

Hi @Harmony321 and welcome to the forums.

Not sure whether I qualify but I have quite a large area on my left lower absomen. can feel a roll of hard material if I pinch the skin around it. It makes the area unusable for injections/cannulae . I haven't injected there for over thirty years but it's still there...:arghh:
I'd be very interested to know if removal of the lump would render that area usable again.... Definitely much bigger than a "small" lump, maybe 10cm long?

I think I have a bit of lipohypertrophy on my thighs too, and the skin seems slightly pitted there, but my buttocks and arms are fine so I have enough sites for injections and cgms.

I would be very interested to hear what your team say about your issues. Good luck.

Thanks for your response!

I am strongly considering liposuction - privately paid for of course because it has genuinely affected my confidence and self esteem. Not sure if anyone has had any experience with lipo? does it help? any side effects? what was the healing like? Just sick of looking at myself in the mirror and feeling disgusted.
