was due my 6 month diabetic consultation in october but the appointment was cancelled and rescheduled for 4th Jan.

Just noticed that last time round i was not given a blood packet to get my h1abc test done a week before.

Anyone any idea how i sort this out before my next appointment?
 
Pick up the phone and ring the surgery. They will sort you out. My surgery never gives me a blood form. It is computerised. I make an appointment to have my blood drawn and when I turn up the health care assistant has the form ready printed out. You need to be quick though!
 
Im under a hospital clinic and Ive rung the medical secretaries for my endo when thats happened in the past - had to go to the hospital out patient clinic to pick up the forms. Guess a phone call to whicher clinic youre under
 
I get given an envelope for my bloods from the hospital, but sometimes it doesn't survive the 6 months between visits. If that happens I just book a "diabetic bloods" appointment with the nurse at my local surgery (GP) - the results are visible to both GP and hospital.
