Accuchek combo...new gizmo

Hi all

Went to hosptial today, and my DSN has a new gizmo from Accuchek so that when pumps and remote are downloaded the new gizmo will recommend suggested changes required for basal rates....

This could be really good for new persons going on to accuchek pumps if all DSN's have them...as they ould have suggested basals and tweaks etc set up a lot quicker....

I have had mine tweaked by the new gizmo today...and DSN knows that I will test the settings to the full.....with testing the BG levels....only changed by 0.6 less for all day, but it has tweaked time period/dosages.....

Has anybody else had this done on their pump? Did it work? Only been 3 hours so far, but my dog walk this afternoon was a brilliant level afterwards...so quite pleased...will let you know how my results pan out.....
 
I know back in January, the pump team (several consultants/nurses) all went on a training day at Roche for new software for pumps! So I can only assume that it was this software you talking about DD

And my consultant when I had my March appointment he loaded it all up to a software program, but I didn't get a good gleg at what it looked like, as he nearly fell off his chair in excitement and started rambing onto my husband about my control :oops: I was sort of left out on this part :(

And all he said was keep doing what I was doing really!

Perhaps next time I'm in clinic which I might find out a bit more, but alas I not there now until the begining of next year!
 
As far as I know this has only recently come out....they've always been able to look at your base lines etc...and my DSN too has been really pleased with baselines and reports etc...(we haven't got a pump consultant at the moment), and our other diabetic consultants do not get involved at all....

It was really impressive for the information on it.....and I think it could just be so much quicker for new pump users to get their levels...although there is a downside of getting them tweaked by a machine--is that the user has to be able to have the confidence and knowledge of when to tweak anyway...

Of course, I shouldn't have to change my ratio's as my actual bolus/basal ratio's will stay the same with a 0.6 tweak (hopefully)....

All the info from the computer is just transferred straight back to the pump and profile all set up.......benefit for me-as my down button on remote isn't working too good...so saves having to get frustrated with it....

That sounds amazing! You're right that it's good for patients to get familiar with changing things like this themselves, but it would make things a lot easier both for new pump users and veterans alike! Too bad my hospital only do Medtronic Paradigm Veos; although they might come out with a similar concept soon? Fingers crossed :lol:
 
OK, I am confused (again!)

Does the gizmo calculate, without fasting data? If it does, then fantastic!

My consultaant just looks at HbA1C and asks about hypos etc. He does get you to email fasting data to him, to advice on tweaks. Mind you, I spend so much time on temporary basal rates it is difficult to get a handle on, any exercise seems to have a big effect on my levels.

I got "well, it seems to be working" as a conclusion. True...but as he remained firmly in his chair I clearly have some way to go to reach Jopars levels :D
 
I suspect what it does, it checks out your basal setting, then uses the duration of insulin, compare's it to the rest of the information, and perhaps suggest a slightly different tweak, perhaps how you've step up or down your insulin might be better being tweaked!

Must admit the more I think about it, the more that I'm not sure, as well can't see how it know enough to suggest something!! But saying that I haven't seen the software!

I've only ever had one suggested tweak from my pump nurse, and that was in the very begining, I'd been pumping for about 6 weeks, and when they looked at my basal setting they felt that I had too much of a jump from one hour to the next one, so suggest that I might be better to put a couple small increases over a couple of hours to reach where the final amount! Apart from that I've sorted everything out myself..

I've had a lot more practice than the others, the more experience you tuck under your belt the better it gets...
 
My consultant gave me info (before not working currently)...about tweaks, and I have tweaked everything expecially due to all the different tablets that have had such side effects as raising an lowering my levels.

The original gizmo at hospital enables HCP's to see base levels, actual readings and whether you are managing to achieve flat lines between the limits that are wanted for us. It also tells them whether we are achieivng the 50 50 ratio between bolus and bolus.

This gizmo yesterday took information from my pump and my readings and other data (I am assuming) and tweaked levels......NOW the interesting RESULTS: now 24 hours since having them tweaked...

My s/d normally runs pretty good between 1.3 and 1.6as a norm....my levels of s/d came in at 1.4 for yesterday and 1.5 so far today (from midnight....).....so not much difference EXCEPT
....my waking day time levels were really tight yesterday they were between low 6's, low 7's and 6.7's at midnight (with a multiwave as well!!!!)...then at 2.30 and 4.30 and 7am I was low.........the gizmo had actually raised my basals for the night -time....so not good.....I dipped a long way down...still achieiving a good deviation rate-which makes my readings of s/d that should be even better really for today....infact my BG's have stayed between 6.2 and 6.9 since I got up this morning.

I agree totally that we should all know how to do our tweaks....but I know that can also seem a way off for people that get DP...like for me set up on a basis of 0.47 for 24 hours...with my DP my highest hourly rate is now 1.50!!! When first starting off in the first 6 weeks it took a lot of frustration to get to the point of coping with the DP and changes...(and of course-it's raised even more now because of my latest tablets...nothing beats testing and tweaking...

It was helpful to me though not having to go in and manually tweak, as my up and down buttons on my pump are less responsive than when I first had it, and I would have got a bit fed up trying especially to get the down button to work!!! One tweak the gizmo did was change the 6am and 7am basal slots round the other way, what I was giving at 6am was changed to 7am and vice versa.

I know my DSN knows I have the ability to tweak everything..myhospital are so pro active with pumps and care of pump users, it was just nice that my DSN's time and mine wasn't taken up on writing anything down or manually keying any changes...it just transferred everything....and so far the day times have worked even tighter...

Night time I am going to follow the pattern of change the computer said, but just lower the basals from what it suggested...
