Simple question hopefully! Should one expect an immediate improvement in blood sugar readings or a gradual improvement, say a few days, if medication is adjusted?

it all depends on the medication, the amount of change and which results. Changing bolus insulin and testing your BG is within 15 minutes but you A1c won't change for 2/3 months, and altering things like basal insulin or metformin can take days or even longer.

Ah I was trying to keep the question simple! I'm on Lantus - Type 2.

I meant if I change say my Gliclazide , does that reflect on the next day readings or does it take a few days for the changes to reflect in the readings?

You might as well assume that it will take days. My DN always says that tablets take a couple of weeks to become established and about a month to go away if I am weened off them. Evidence suggests she is correct.

In addition, if I make a dietary change (I experiment a bit) it can take up to a week to go from here to there.

EDIT: Here is a picture of me drying out. Readings taken same time every day.

9.5, 7.7, 6.8, 6.8, 5.9, 5.3, 5.2, 4.7
