Hi im new to the forum, but not to diabetes. I am type 1 now for 35 years. At the beginning of august, I had three bad hypos in 24 hours, two of which required help from the paramedics, which led me to the hospital. I was advised not to drive due to hypos by DSN. i had been having lots of hypos (at least 1 a day) with no pattern to my blood sugars at all, and very little hypo awareness, not to mention very lumpy injection sites. Since then i have been learning to carb count, have changed fast acting insulin to novorapid, from humalog which i was on for more than 20years, but blood sugars are still doing their own thing, still no pattern!!! So im wondering if a pump may be my way forward? I have read the nice guidelines, and think I fit more into the ABCD guidelines, well 2 or three of them, has anyone else had similar experience and gained funding for the pump, or is my Consultant going to say no??

Lipohypertrophy problems can lead to erratic insulin absorption and may explain your frequent hypo's, however the usual advice when bg control goes astray is to start with some basal testing and then work on your I:C ratio's if your basal is OK.

As to your pump query, yes you do fit the ABCD guidelines as these issues are impeding on your quality of life, plus hypo unawareness (or starting to lose your awareness symptoms) fits in with the NICE criteria, I would certainly ask your Consultant about the possibilities of a pump and see what they say.

Thankyou for your reply noblehead . I do have lipohypertrophy, (now I have read the definition, I just called it lumpy injection sites lol), not surprising after 35 years of injections really, my DSN has told me not to use my left leg at all, my arms and bottom are not much better. As for basal testing, I'm doing 4 glucose tests per day but not including during the night so will give that a try. My lantus dose has been greatly reduced since the problems began going from 38 units to 24 so maybe DSN are already looking at basal??

I will ask my consultant about pump therapy, next month when I see him for sure, well if you don't ask then you don't get right?

Hi Hirstygirl, I have been type 1 for over 53 years,over the past 3 or4 years I have lost my Hypo awareness and had no problem getting funding for a pump,which I have been on for 4 months.i dug my heels in and with help of my Endo and DSN won funding for a CGM,which took a little while to get but under the new NICE guideline should not be so much of a problem for you.Since being on my pump and CGM my problems have almost gone away.

Sorry it's taken so long to reply to you yingtong, I'm waiting for an appointment with pump specialist have been told it will be during the first half of this year, in the meantime I'm changing on to tresiba next Monday (been reassured this won't affect my wanting a pump, but may help sort bs out)
I'm looking at the Medtronic 640g and would be happy to learn more from you, if you don't mind sharing, about getting funding for cgm
 
I was on Tresiba before getting put on a pump, it is a fantastic Basal insulin and the Consultant wasnt going to put me forward for the pump as my A1c was very good, But this was masked by many lows and highs and not totally loosing hypo awareness but not getting symptons when I was dropping until I was at around 2.2mmol. I was testing over 10 times a day on the run up to the pump appointment so he could see what was happening and he agreed the pump was the best option.

I have only been on it 5 weeks and already getting more awareness back at around 3.4mmol but if it has fallen real quick (when I'am real busy at work cooking) it can still drop to low 2,s which is still a massive worry with me being a chef and so close to deep fat fryers, griddles and the likes but hopefully in time I might get more warnings back, good luck at the appointment, I will keep my fingers crossed for you :)
 
I was a little worried that I would lose my place in the waiting list by transferring on to tresiba tbh, but I have agreed to be part of a research group whilst using it and got the consultants promise that I will be considered for pump at my hospital in the first half of this year, in the meantime I'm happy to try anything that might sort bs out

Wanted to say well done to gazm in 5 weeks you have some hypo awareness back, that is just what I need ...... Still waiting tho
