I wanna know what is the hard moment for following and be persistent with the Diet provided by a Nutritionist. Thank you !

probably sticking to a routine keeping blood sugar in check and within range, usually dietician/nutritionist will advise on small changes to your 'normal' diet cutting out unnecessary items like chocolate, cakes, biscuits and junk food. personally i find it hard to resist temptation on falling back into bad habits as I still eat processed foods, crisps, pizza more often than i should

We all have different strategies. Some will suit your temperament too:

See these foods as poison, not treats
Don't buy them then you can't eat them
Find foods you love that are diabetes-friendly
Buy and eat those as much as you like
It isn't a competition, and you don't "win" by eating foods that will make you ill
It's a serious illness, so respect it
You have control over your lifestyle, so be proud to control it

I'm sure others will be along soon to add to this.

Food intolerances make you ill. If I eat anything containing gluten or lactose I suffer. In fact, death is preferable to the pain of IBS. So I don’t consume either. It is therefore easy to follow a non gluten / lactose diet. If you consider T2 as a carb intolerance, then that too makes you really ill, but it does it silently. It’s easy to ignore until you reach a point where you become very ill with diabetes complications. Without the immediate pain of intolerance to remind us just how dangerous sugars are to your system it is easy to stray off your diet. It takes strength to fight a silent battle.

I've been a life long migraine sufferer, and have had a few food triggers, so learned in a very painful way that I should avoid these. When I was diagnosed with T2, and could no longer handle carbs very well, I essentially treated carby foods in the same way as I had with the migraine triggers - with the caution they require.
