Basal Bolus

Hello guys so just yesterday my diabetes nurse switched me over Lantus and Humalog from Mixtard 30 and actrapid since I had dawn phenomenon and Mixtard was unable to combat it... anyway my sugar before bed was 350 mg/dl (that's likely to happen because I'm new to this regime and still figuring out my doses).. I took 12 units of Lantus and 3 hours later it was down to 290 mg/dl then 3 hours later it became 203 mg/dl then at 8 o'clock it went up again to 272 mg/dl (still on Lantus at that point and I haven't taken my morning insulin yet at 272 mg/dl)
Do you guys get this decrease by 147 points just because of Lantus? Mind you the same day I took Mixtard 30 in the morning and today is my 2nd dose of Lantus but could any traces of Mixtard have done that? What do you guys aim fir at bedtime and how does it change?

Hi. Basal/Bolus should be a lot better than a mixed insulin once you carb-count the Bolus and the Basal balance right. All T1s should really be started on Basal/Bolus now and not mixed insulin. Lantus is one of the common Basals and Humalog is also common so you should see god results once you have things balanced but you must carb-count

Sure but from your experience how many points it goes down with Lantus?

It's impossible to say I'm afraid. All the Basal does is to balance the background glucose output from the liver and it's the Bolus that probably has the most effect. If you get the Basal balance right so your blood sugar stays steady when you fast for a good few hours and you carb-count for the amount of carbs in the meal you can achieve an HBa1C in a good region. So, in summary it's the Bolus that helps most with blood sugar reduction but as you may realise it's always a game to avoid either hypos or high blood sugar. You also need to keep the carbs in the diet down to minimise the insulin you need and help reduce blood sugar swings

Blood glucose before bed yesterday: 350 mg/dl
*Takes 12 units of Lantus*
Fasting blood glucose at 6:12 AM: 122 mg/dl
Can you explain this drop? It was definitely caused by Lantus

Today is my third day on basal bolus. Last mixtard injection was 2nd of January in the morning. Anyway I'm eating half dinner than usual. My dinner consists of usually one loaf of pita bread but I ate half because I didn't want to run out from insulin which happened this morning thankfully after my shot. Could this explain?
