Ill informed people!
Carried out basal test last night on daughter (aged 6) we are LCHF and last meal was lunchtime with a high fat 3g cho & 3g protein snack at 1830, BG was 10.5 (running highish all afternoon) 2 hrs pp she was 9.5mmol 2215 4.5mmol, 2315 5mmol, 0019 9mmol then raised and dropped throughout the night waking up at 16.2mmol.

Obviously basal rates are not right and our paediatric team don't understand the whole LCHF thing, can anyone offer any suggestions/advice - I am at my wits end at not being able to get this right. Thanks!

What make of basal insulin is being used? And what time us the basal injection?

Sorry forgot to mention, we use a pump and she is on Novorapid

Brilliant.

The basals are needing to be raised.

Are you happy enough about changing basal rates?

I tried and I think that influence last nights experience.
Consultant made the following increases:
0000 0.125
0200 0.100
0300 0.075
0500 0.050
0700 0.125
1900 0.275
2100 0.250

this resulted in hypo's at bedtime followed by hypers around 1am. I tweaked over a period of a week and ended up with:
0000 0.125
0200 0.050
0700 0.075
0800 0.125
1900 0.250
2100 0.275

I was maybe doing one correction when I went to bed and she was always within range when checking at 2am but sometimes hypo or slightly hyper on waking.

What pump has your daughter got?

I eat low carb but not high fat. I think when adjusting basals for newby pumpers, its best not to eat a lot fat as that will just make adjustment to basal a bit difficult when you are not sure what you are doing. Also, adjusting basals is a bit trial and error so you have to be prepared to make changes as and when you need to according to the bg results you get.

it must be hard doing the miss a meal basal tests with a 6 year old, they don't like being hungry

the only thing I could suggest is using coconut fat bombs to help the hunger and have minimal effect on the BG

Surprisingly it was very easy, she had a bulletproof hot chocolate and a cream cheese/coconut oil pudding type thing.
Thanks for the fatbomb link :-)

this could be why your HCP are having trouble with LCHF, normally they just bolus for the carbs. on a low carb diet some bolus for the carbs and then a square/dual/combo bolus for about 50% protein.
I don't abide with the low GI stuff from the link, but I'm T2 and I find the fats make the small amount of carb, everything low GI
