BG Level Rising Early Evening

Hi, I'm looking for some advice about my 5yo daughter who was diagnosed around 10 weeks ago.

During the daytime, the BG levels are pretty good and tend to stay within 3-10 most days most of the time. However, recently the BG levels have started to rise around 7pm and keep rising up to around 15 before dropping back to within target around 2AM.

She is on the MDI regime with 3 units of Lantus which is taken every day at around 7pm. Dinner is at 5-5:30pm so she should be clear of that. Initial thoughts are that the Lantus is not lasting 24 hours and is not active at the time she is going high. It's only when the newly injected Lantus becomes active that the BG levels seem to come back down. We have considered increasing to 4 units of Lantus but are concerned that this will lead to early morning lows or even daytime lows.

She can occasionally run high during the day but there is no pattern. The only constant high at the moment is the one I have mentioned between 7pm and 2am.

I too think it might be the Lantus running out. I've never used Lantus and I'm on a pump so I can't advise, but I would speak to her DSN and see what they suggest. Perhaps a split bolus or moving the time? Or even changing to another basal insulin?

I agree with @azure - I think it's the lantus as well. I take my lantus dose at 8.30 pm every night but like you've noticed with your daughter, my sugar levels are higher late afternoon - early evening. I've heard some split the lantus dose. I'm seeing an endocrinologist though and he insisted I wasn't to split the dose. But it definitely does not last 24 hours on me that's for sure. I just wind up using more bolus insulin later in the day to try and control my sugar that way.

It might be that your daughters lantus is running out, some adults have to split-dose their lantus for this very reason but don't know if this recommended in small children, all I would suggest is you speak with your daughters diabetes team and mention the evening highs.
