Hi all, looking for advice. My 8yr old daughter is experiencing hypo's twice a day before lunch and before bed. We use the accu-chek expert so I thought we were being given accurate info from machine. The DSN told me to now override the machine if I think in my opinion that it is going to give her too much insulin. She said for example if it is telling me to give her 14 units then I should make the decision to giver her 12, and if she is being given 6 then I should give her 5. What do I know???? We are only 6 mths into this craic and still learning. She said we can play with about 10%. I don't really want to play with anything. Anxious that I am now going to replace hypos with hypers as she will not get enough insulin. When I checked Dr Ragnar Hanas's book he said that if it is a hypo before lunch then it is more likely to be the basal not the bolus causing the low. Anyone out there who can help please? Mx

Do you test your daughter's bg level mid morning at all? If it is lowish, then it might be that you need to reduce the bolus a tiny bit. Another thought, is that bg levels can lower at odd times but one way of preventing the hypo before lunch is to eat a small carb snack mid morning and then test again before lunch to see if bg levels are ok again.

Years ago, it was thought that if hypos occured within 2hrs of eating lunch, then it meant that the basal needed to be reduced, but this was when Lantus was being used once a day. When diabetics started to use twice daily basals, then a different ball game started to emerge where people just altered their bolus - insulin to carb ratios.
 
Hi, she tests in the morning takes her bolus and basal at roughly 8 am. School has snack at about 10:45 so she eats then. She can either have a hypo at about 9:00 if she has had a big breakfast and a large insulin dose ie weetabix juice and milk or it is just before lunch even after she has had a snack. She always tests at lunch. We have adjusted her ratio for the breakfast again to try and prevent this. Another problem is that she is losing weight so I need her to eat and don't want to discourage her eating a good breakfast. What do you think?

If your daughter also injects the basal at breakfast, then yes alter it down just a tiny bit (1 or 2u) and then hope that the hypos before lunch start to stop.... This should sort things out for you both with any luck........
