Blood Glucose Monitoring Booklet?

I was just wondering if anybody has any information on where I can get hold of some booklets which let me input my Blood Glucose readings so I can monitor them more easily. When I was younger, my Diabetes clinic used to hand them out for free, but I'm not sure if they still do this.

I need an easier way to keep a record of my blood sugars than having to look through my machine's memory all the time. Even if there was a sort of spreadsheet online which I could download, that'd be good.
