Type 1 Blood sugar spikes early hours

Can anyone help with an answer - every early morning my blood sugars start to rise around 1-2am, I generally go to bed around midnight with a b/s of 12, for this I give myself 1 unit of humalog insulin - I’m currently on a pump and my basal rate from midnight to 12 noon is 0.20u ph then from 12 noon to midnight basal is 0.30u - my last meal is 7pm with nothing after this - can anyone offer advice to resolve these early hour spikes

Hi @derekb, How long have you had your insulin pump? And what brand and infusion sets (cannula and tubing or not)?
Who set up up with this programming on your pump and when was it last reviewed. ? Do you do carb counting?
All your BSLs are out of range, not just the ones at night.
As a comparison, my basal rate on my pump was 'sculpted' by my specialist to best match me. The rate changes every hour through 12 mn to 7 am, twice from 8am to 3 pm and 5 times between 4 pm and midnight.
The bolus doses are set up as : pre-meal with an insulin to carb ratio which differs between morning and evening e.g. mine is unit of insulin to 6 g carbs before breakfast and one unit to 8 g carbs before dinner so 3 meals per day with bolus before each.
correction doses for high BSLs (such as the one one unit you give at 12 mn.)
You really need to get your pump settings reviewed as soon as possible. by someone who knows what they are doing maybe be re-educated about your pump plus also see that doctor about other possible contributing factors as such as recent illness, care of insulin, cannula sites etc.
Best Wishes and please do not delay - you have the one of the best means of controlling BSLs but it has to be set correctly for you and you need to understand what to do to keep things working as well as possible.:):):)

Not had pump long - nightmare with getting advice from DN’s they don’t seem to know how it all works - suppose this differs from boroughs - I have the omnipod pump which uses a small cannula - I also carb count - here is my pump ratios - to be fair my blood sugars have been bad for long time, but I find with waking up high it takes until dinner time to get back on track

Hi @derekb, Apologies if I sounded too critical. As far as I know others on site using an Omnipod find they work well.
I could not open your .jpgs.
Could you try the "Upload a File" option next to "Post Reply", Right bottom corner of the posts, if you not already tried it? ?
Maybe could you give us some info about your diet as well?
Working out a person's basal rate for a pump is a real art and skill and I would be asking for an endocrinologist to be consulted. DSNs who are struggling with pump settings for a patient must have some access to a specialist and not be too proud to consult them.
If you are doing the go-slow on obtaining the specialist, or refusing to do so, there maybe other steps you could take*.
But you are likely to be feeling pretty rough on the above figures.
Love to see the ratios for comparison. If there is still trouble one of the Moderators might be able to help or obtain help.
*I do not live in UK , others will have a better idea for you how you can proceed.

I don’t mind critical - as a long term uncontrolled diabetic it may be needed - I do feel rough most days which something I want to resolve - busy work life does not help - many thanks for your input - I think a change in endocrinologist May help

I don’t mind critical - as a long term uncontrolled diabetic it may be needed - I do feel rough most days which something I want to resolve - busy work life does not help - many thanks for your input - I think a change in endocrinologist May help

Hi Derek, good luck to you, but remember a 'busy work life' must still come secondary to your health, no matter what. Health not right and your work will suffer anyway. You are clearly working hard at it and that is a good thing. x

Hi @derekb, Really glad you posted on the forum. The correction dose set up is very basic as is the I:C ratio compared to pump set ups I have seen - my own, some on posts ect.
Most on multiple injections of insulin have more detailed ones.
I was not sure whether an endo set your pump up and the nurses were supposed to help fine tune it for you?
Finding an endo who can do much, much better asap sounds like the way to go.

With the right advice your blood sugars could be much better and you could feel much better. And feeling better makes it easier to withstand the busyness of work and feel m ore able to enjoy your leisure hours !!
