Budgets/prescriptions and availability

Does anybody know how GP surgery's are paid?

I mean, are they paid for every patient on their books, or paid per appointment. Also, who pays for the meds that we receive, do each surgery have a set budget allocated per patient, or do they recover the cost directly from the NHS.

I was just wondering, because going by some members on this forum there is a diference in the quantity and availability of meds for people living with diabetes.
