Hi, I was diagnosed with type 2 a year ago but have been unable get it under control despite numerous different medications, including Metformin and glimepiride, and now Lantus. A C-peptide test was ordered + GAD. I phoned to get the result and was told the C-peptide was 0.45. Does anyone know what that means or whether 0.45 is high or low? Thanks.

Hi. I think you will find that is very low and indicates that you are probably not T2 but LADA. The tests vary a bit but my 'normal range test was 1.1 to 4.4 and my result was 1.11. Looks like you may need to go onto Basal/Bolus regime i.e. have a meal-time insulin added if necessary and the Glimepiride then stopped? Do discuss with the DN.

Hi,
Different countries use different units when reporting. Uk uses nmol/L and US use ng/ ml I believe.
My own results were .48 nmol/L which may me a bit low.

Below .2 nmol/L is generally considered absolute insulin deficient. But up to about .36 may also be insulin deficient from some articles I read.

A group of 24 healthy individuals, continuously monitored for 4 days in Denmark had a mean c-peptide of .59 .

Significant insulin resistance usually equates to numbers above 1. A study which followed 16 expecting women 3 months before and after birth found mean values of 1.1 in final trimester. Insulin resistance is normal in the later stages of pregnancy.

I don’t know if that’s any help or not...
