Hi all,
I’m new to this forum and this is my first post
I was diagnosed as diabetic in February 2019 and was put on Metformin 500mg 3 times a day. In September 2023 my A1c went up to 83 so my GP put me on Syjnjardy 1000mg metformin and 5mg enpagliflozin twice a day. Since then my A1c has dropped to 62. No advice of any side effects was given or any lifestyle check was done. They just changed my medication.
Looking around the prevailing advice seems to be that a low carb diet can help control my blood glucose level. I had tried low carb and intermittent fasting before and was fine with it. So in September 2023 I put myself on a strict low carb diet coupled with intermittent fasting to try and get my glucose levels down.
I crashed badly and had symptoms of extreme fatigue, shortness of breath, abdominal pain, lack of appetite, and vomiting. My GP put this solely down to my high glucose level. I wasn’t satisfied with this answer and as I have private medical insurance through work I sought a second opinion. The private doctor checked my liver and gallbladder function and ruled that out. He said it could have been costoconditis caused by a viral infection. At the time I had stopped the diet and was having Complan to get my appetite back. My GP said he would prefer me to fast instead to get my blood sugar down.
Fast forward to 20th April 2024 and I thought to try again with the low carb diet. I felt well enough and was under the impression that a viral infection had caused all the problems in September.
I crashed again with the same symptoms. So I put 2 and 2 together and researched if a low carb diet coupled with an SGLT-2 inhibitor can cause any issues. I was shocked to find there are published medical papers with patients experiencing the exact same symptoms. With other doctors saying that low carb and SGLT-2 inhibitors isn’t recommended as it can cause Euglycemic Keto acidosis. I was never warned of this and there is nothing stated in the medicine info leaflet.
I’ve been to see my GP today because I really don’t know when and what I should be eating. He said I should minimise my carb intake and should read through this website as he doesn’t have time to give me any information. I queried what minimise my carb intake means as I don’t want to lower it to a level where I crash again and again he said read this website.
I feel lost as I don’t know when and what I should be eating. Has anyone else had this happen to them and what sort of diet do you follow now?
Really appreciate any advice anyone can give me.
