Changing to Degludec insulin

My doctor has just prescribed the new insulin Degludec (Tresiba), changing me from Lantus, as my diabetes control has suddenly gone haywire, and my Hba1c has gone up. I've been diabetic since 1975, and amazingly have had good control up until now (bar a while in my teenage years no doubt). My Lantus dose has increased by 18 units over the last year, as has my Apidra, but I just couldn't get it balanced, which is unusual for me.

I would welcome comments from anyone who has changed from Lantus to Degludec, and also dosage comparison. I am currently taking 38 units of Lantus, and my GP has suggested taking 30 units of Lantus, but I'm wondering if this is still too high, seeing that it has a 40 hour duration time.

I am also a little concerned about possible night time hypos, as Degludec supposedly limits night time highs, as it works for longer.
 
I was on 12 units of Lantus and started out on 9 units of Tresiba, but have now settled on 10.5 units. I know Tresiba lasts a long time but you should take it daily at the same time like you did with Lantus. Also wait 3-4 days between making adjustments to your dose to give it time to bed-in. I have had far fewer night time hypos with Tresiba once I got the dose sorted.
 
How does this impact on your weight as Lantus makes me gain pounds very fast
 
I never had a problem with weight gain when on Lantus, so I couldn't say. Perhaps your dose is a bit high and you are feeding your insulin?

Wow, you're on a low dose, that's good. I wonder why mine has increased so much. Thanks for this, I will persevere with it.
 
I'm now at my all time heaviest, having put on two stone last year :-( I hope the new insulin will help with this, along with slogging it out at the gym!
 
I've been using Tresiba for 2 months now and just today noticed I've lost 5 pounds during that time. It's probably not only because of changing Prothaphan to Tresiba, but I guess that has helped. Less hypos = less carbohydrates. Hope it works same way for you :).
 
Hi, I was on Levimir (26 units) for a number of years but I started getting "ballooning) on my injection sites for some unknown reason, and was put on Lantus (a straight swap 26 units) just before Christmas.

After a series of bad hypos and uncontrollable blood sugars I dropped the dose down to 14 units but I was still having problems controlling my over all bloods.

I was put on Degludec 2 weeks ago and was told to start on 12 units, I had to increase this to 14 units and not had a night time hypo as of yet, BUT no matter which way I try changing my morning insulin dose I,m having a hypo every afternoon at work. I have to phone the diabetes specialist nurse this week to discuss what effect the Degludec is having.

I have had type 1 diabetes since I was 2, 43 year ago and over the past year not had the good control have had most of my life.

This maybe down to the length of me having this condition, I will post again after I speak to the nurse

Have you done a basal test since switching over?

It might be that your bolus ratios have changed since swapping to Tresiba but you should do basal testing first.

I carb count, so the bolus amounts are never really the same, tried it 1 unit to 1cp, 1 to 2

think they will probably put me on constant blood monitoring for 5 days on my next visit, a pump has also been mentioned several times but nothing has came of that yet

No you need to do a basal check before looking at your I:C ratio's, read the link that Robert provided above.
 
I,ll give that a try, won't be able to do it any time soon though as I,m a chef and work alone and don't want to make things worse

but will do it when I,m off work

Its better than It has been, but I've got problems with the 20 hours that Lantus works for sending me high at some points and when it 'kicks' back in, it sending me low.

Just wondering what people's expreince is of Degludec, my diabetes nurse is not wanting to make too many promises because she gets good results from some but bad from others. The whole 24 hour flat action sounds great, but I'm just wondering if that is the experience of people who have been using it?

Hi Dave - I've been using it for a year now and find it very flat. I don't have any complaints.

I also found Lantus to be a bit variable and not last the full 24 hours.
