confused about my insulin dosage

I have been Type 1 for 10 years, and all was well until I went back to work, about 18 months ago, since then all has gone haywire and I cant figure out exactly what I am doing wrong!!! On my days off of work I take 26mm of Lantus in the morning which keeps my BS at 9ish all day, but I drop 10 over night, so need to go to bed on readings of 17+. On a work day I drop the Lantus to 20mm which in turn leads to high sugars all day ( I work in the evenings) but only drop 3 or 4 over night so feel comfortable going to bed on 11's. I work every other day. My problem is I have completely lost the plot and cant figure out a way of keeping a good balance all the time. I seem to either run high all day or hypo at nights and cant work out how a happy medium with my Lantus. Any help would be gratefully received!!!
 
Can you tell us a bit about your bolus insulin (presumably Novorapid or Humalog)?
Do you carb count?
On the work days when you only take 20units of Lantus, how much is your blood sugar increasing every hour?

Since you're only taking Lantus in the morning, it would seem most likely that it isn't just your Lantus that's causing the issue, but rather the combination of your basal/bolus insulin.

edit- I looked through your past posts and it seems you had very similar issues back in 2012 and 2013. Can you share a bit about what worked during those years?

The best thing I would suggest contact your diabetic nurse as she would be able to guide you with the right dosage. Going to work is a change that the body has to adapt and I'm sure the diabetic team would help you.

I use Novorapid. I was having similar problems 2 or three years ago, and saw a diabetic specialist to try to work out where I was going wrong. I also did a carb counting course and gave up work in order to have a more structured life style. all this helped initially, but since I returned to work all has gone haywire again and I cant seem to stabalise my insulin regiume.

Unfortunately I don't get great diabetic care. On the last two visits I made for blood tests the nurse didn't even understand that I took insulin until half way through my appointment, despite knowing I had been Type 1 for 10 years, so I don't feel overly confident with their advice.

Do you see a consultant too?

I find my GP practice nurse isn't that great, but the consultant and my DSN at the hospital are very helpful.
 
Sweep, hope you don't think I'm being rude but are you type 1, or insulin dependent type 2? I note that you are 54, and although it's possible to have late onset type 1, it's not that common. Were you type 2 that then started on insulin? If so, you are still type 2?
 
I had a problem similar to this after coming back to the uk from being on holiday. My bg was monitored continually for a week and it turned out I wasn't having quite enough lantus. I would go to bed around 6 or 7 mmol's and wake at 15. I split my lantus so on my consultants advice just upped my dose by 10 percent (actually 1.8 mmol's ) and this did the trick. However I keep my lantus dose to a regular amount not changing it like you do. The body doesn't respond instantly to dosage changes it takes from a few days to a week to adjust so changes should be made gradually. What is your carb/ insulin ratio? Maybe this isn't quite right either. Also stress can make bs high. My bs is usually higher at around 8 or 9 until I get home from work when it drops to around 5 with no correction does.

Can I ask if your under the care of a hospital diabetes clinic? If your under the care of your gp surgery then ask for a referral, hospital clinics are more specialised in dealing with type 1 issues than a gp surgery.

In the mean-time start with some basal testing to get your background insulin right, you shouldn't be going to bed on a bg level of 17 because your bg drops 10 overnight, when basal insulin is at the right dose your bg shouldn't fluctuate by more than 1.6 when fasting.

Type 1, bit biazzar really was cos I went into hospital for day surgery for something else and was immediately diagnosed with Type1....basically inject now ore you cant go home!!
 
I do carb counts. I'm on both long acting and short acting insulin.
It's different for everyone.
I think what you need is the right professional to guide you to decide what works out best for you.
