Hi very long sory, T1 Diabetic over 36 years, control ok but could have been bit better. Had hysterecomy last May, various infections, only IV antibiotics would work.Got various probs with wound.. hospitalisation twice then again in August for septic shock (had peranal abscess) On High pain meds due to calcification and fibro. Since then have had infection, virus, fibro flare up etc ocnstantly. Have yet another viral infection... viscious circle.. high sugars... prone to infectin.. more high sugars etc etc, Can manage with LOTS of extra insulin but dose gone up sooo much dont know whats normal. ANYONE ABLE TO HELP??? Sorry for maon but am at end of tether. Lost good friend to diabetic complications last march..dont want to be at risk with constant high sugars. (aug mega high @59, T.g.i recovered. At mo high teens but can get the to about 10 with lots of insulin. Specialists say increase insulin as necessary but find out cause of highs. GP says go to your specialist!!!! :( Meant to say had come off levemir and lantus and now back on insulitard with better control when not sick, thinking of changing frm NOVORAPID back to actrapid as these insulins dont seem to be working or my body is too used to them . On average 140 novorapid and 100 insultard daily at mo

Morning Jinty..

Ok a couple of things that I am going to suggest..

Firstly the amount you are injecting is quite large.. it's a bit hard to access without knowing your weight and general activity level but 240 units a day is quite a lot.

I would suggest that this is being caused by two things:

- Firstly illness clearly increase our need for insulin as the liver dumps glucose
- Secondly increased blood glucose reduces the effectiveness of insulin.

To manage this I would suggest the following:

- Reduce you carbohydrate intake to a minimum preferably sub 60g / day. This will make your blood glucose easier to manage.
- Aggressively manage those blood glucose to sub 8 mmol/l
- If you feel well enough introduce some moderate exercise.. perhaps 30 mins walking a day..

If you can reduce them down and keep them low then I suspect your requirement will begin to drop.. make sure you keep testing regularly to avoid possible hypo's.

In some of my diabetes more irritating moments I have injected 150 units + of rapid acting in a night when my sugars for no apparent reason have gone mental... normally I would inject no more than 35 units in total for the whole day.. high blood glucose can massively increase your insulin requirement, the only way to stop this is to be aggressive with your management until you get them back under control. This does carry hypo risks as at some point you will tip the scales and then your bloods will drop quickly! so please test, test, test.. probably at least once / hour whilst you initially bring them under control... more frequently if they are dropping at more than 2 - 3 mmol / hour.

Thanks for the advice, i'm unfortunately overweight and in poor physical health, am getting physio for calcification in joints and heart. This is obviously causing problems. I am eating very little at the moment approx 60 carbs a day, mostly fruit & yoghurt.
also taking oxycontin 30mg twice daily and baclofen 10mg 3 times a day. Will try hard to do a little exercise. Thank you. :thumbup:
