Daughters first night on insulin

Hi
My daughters went insulin yesterday. The hospital had forgot to advise us to stop the lantus the night before so they had to make some adjustments and have a 6hr temp basal rate of 0%, also further complicated by Sophie's level being HI as we switched over so they needed to correct it as the correction dose we had done on the way had done nothing at all.
Level dropped to 24 after 45mins and to 8.1 after another 2 hrs. 7.1 at 9pm, 7.5 at 11pm, 3.3 at 3am (basal reduced to 1.1) 3.9 at 6am (basal reduced to 1.0) 4.6 at 7.45, and have sat between 4.2 and 6.2 all day.
The DSN phoned earlier and seemed really pleased, just suggesting we reduce basal at 3am and 6am if it drops again tonight. She doesn't think this will happen as we've made the reductions already. I'm not so confident.
Sophie got a bit worried at school as she was convinced she had air bubbles, i suggested as it was near lunch we wait and see what her levels were and just keep an eye on the levels to see if theyr causing a problem. Fortunately they didn't, I think Sophie was just a bit anxious.
Fingers crossed for tonight, I hate her going low.

Are they really good results, I know the DSN said so too but I was disappointed with the lows.
Also, forgot to ask, how long do we need to do this 11pm, 3am, 6am checks - I know the obvious answer is going to be until things settle down!
Is there an obvious reason for 3am and 6am, because that's when your more prone to lows?

Just read your info about your daughter, hope she is ok and does well with the pump.
I go on my pump this friday 25th November, I read in your info that they should of told you to stop taking the lantus the night before.(your daughter)
i take 24 units of lantus every night before bed time (10pm) the DSN has not told me to not take my lantus on thursday night, should I contact my DSN and ask her if I should not take my lantus on Thursday night :?

I think our DSN was wrong, she looked very uncomfortable when she realised. I read somewhere in the training docs that lantus would stop but we never discussed this at the first meeting when Sophie went on saline.
It took Sophie over a year to be convinced to try the pump so the manuals were quite old so we didn't really discuss them again at the training. I feel a bit guilty too now, but your trying to take on board so much information and instructions.

thank you for your quick reply, I have got the ACCU CHECK SPIRIT COMBO at home which I will take with me on Friday.
When I was told that I could have a pump, I rang ACCU CHECK and asked them to send me a manual, I have read it over and over again but still a bit confused over some of the things. My wife is comming with me on friday and I,m sure she will be on the ball, I will keep you up to date on how things are going. I am going straight onto Apidra insulin not saline. I have been inject Apidra insulin for the last 3 years but am confused why they have not started me off on saline. :o
What pump is your daughter on ?
As you can see I have been a type 1 diabetic for 50 years now.

those results are very good yes there were a few lows in the night but u dont expect things to be perfect right out, the 8.1, 7.1, 7.5 was perfect she was holding stable in a general area so sounds like ur in the right area with the basals except for the night time obviously.

i found with dylan that once the night was sorted the day slotted in much easier, i think they ask u to check at 3 and 6am as those times are just certain points through the night to make sure she isnt hypo it could just as well be 2 and 5 but i think 3 and 6 are kind of mid way. i remember those early days i felt like a zombie i hardly slept at all even between those times as was worrying to death he would hypo. the other thing i found hard to get my head round was the only give fast acting for hypos it was so in built to give fast acting and then following it with a slow acting carb but it does work.

as for the long acting insulin before going onto the pump we were told to give the full dose the morning before then give half the amount the evening before going live with insulin so we still had to consider the active levemir in the background but it was no where near as much as it would of been.

things will soon settle down and u will wonder how u coped before the insulin pump dylans is our new best friend we love it and wouldnt be without it

I go onto the pump tomorrow at 9am, straight onto Apidra insulin.
My friend (who is on a pump and is a type 1 diabetic) phoned me at work today, and said did you know that you do not need to take your lantus tonight, when my friend went on his pump he was told not to take his lantus night insulin.
I contacted my DSN today and she has told me to take half my dosage of lantus, this will be 12 units of lantus to take tonight. :?
I am looking forward to going on the pump and hope all goes well for Dylan and Janerd daughter. :D

Sophie's 3rd day and all going well. No lows today and average levels of under 9. She was in the high teens mid 20's pre-pump. Amazing! And she already feels much better for it. Not thirsty all the time, less irritable. Hope things go as well for you x
 
Well done Sophie too. Day 10 and my levels are between 7-9 most of the time and they don't dip or climb drastically like they did on injections (we've still to get the morning numbers right from about 5am till 10am), but slowly tweaking it with my diabetes team every other day is a good way forward. I also did a full set change tonight complete with filling the reservoir and although I still find it a bit daunting it gets better an easier with every go. Let's hope this continues for all of us and that it all goes well for you tomorrow Kev. Riri x

I am glad sophie is doing so well, Jenard you must be an angel.
I can remember when I got my diabetes at 6 years old my mum did not understand it, I lived in hospital for 2 years then when I was 8 years old I was moved into a diabetic hostel until I was 13 years old. I cursed and swore at living in the hostel but I learnt so much about diabetes and I am still learning.
I have my years of problems with the diabetes operation after operations, I have been testing my blood 10+ times per day and injecting up to 10 times per day. Like you Riri my sugars are up and down like a ping pong ball, I take both my blood testing kit and insulin to bed with me, I dont sleep very well and check my blood through the night to the morning when I wake up, my sugar say at 2am can be 10-17 so I inject insulin this happens to me most nights even though I go to bed with a level of say 5-7. I am hoping that the pump will improve things for me in time. Anyway I wish you all the best with your pumps and I will keep you up to date with mine. :thumbup:

Hi All
Just to let you know I am now connected up to my Accu check spirit pump running on Apidra insulin, the trainning on the pump last about 3 hours, so far I have had 2 low blood sugars today and no highs yet, I have the night time to go through but as I dont sleep very much I shall test my blood every 2-3hrs through the night.
The spirit is wireless and I have some software which the REP gave me so I can do all my adjustments and then send it to the pump. :thumbup:
Being an electronic/software engineer by trade, I can see that Accu check has designed a good pump :clap:

Hi Kev,
Sounds like its going well. Sophie had 2 lows after setting up the pump. Things have been a bit up and down since. On the whole it's been very good, she has had a few lows during the day and now the DSN says she is starting to look for patterns before she makes any big adjustments. Hope thing continue to go well for you.

hope Sophie is OK, I know what it is like when you are very young with diabetes.
Well its not too bad at the moment as you can imagine it was around midnight (25/11/2011) that I went low 2.7 :thumbdown:
took 12g or sugar (Blood meter told me too) at 12.30am it read 4.7 (26/11/2011) off to bed as I was so tied :yawn: at 7.30 am my wife done blood test for me as I was a sleep it read 1.9 :thumbdown:
she woke me and gave me 20g of carbs by 8am it was 6.4, at 9am I had 5.4 units for 30grams but since then its went up to 16.9 but I keep testing and pumping insulin and now its starting to go down :thumbup:
Like my DSN she wants me to email her all the info by the end of next week to se whats going on and do any adjustment. not a bad night just have to get used to not using a syring which i have done for 50 years now.
My wife is so good with this complaint, i call her my little nurse and could not do with out her, just like you jane a loverly nurse/ mum to Sophie.
my love to both of you and give sophie a big kiss from me for working hard with her diabetes. :clap:

Hi Kev,
You sound exactly the same as Sophie during her first few nights. It was only the first night that she went low and now we are seeing a rise in the morning. The DSN said she expects to see a pattern of higher levels around 5-9am, which is now happening. 15.9 at 6am and 16.4 at 8.30am. Then as the day goes start to see the lows! I think the run of lows is starting to make Sophie feel tired and groggy, the hospital thought this would be the case. Let's see what Monday brings. Really hope you contine to do well and the pump brings you all you hope for. Keep in touch x

Hope Sophie is feeling a bit better and not too groggy :thumbup:
I read your notes about the highs and lows and I am not surprised either at Sophies results, I think diabetes is one of the hardest things to grasp, like when you have a tea/coffee with milk do you count the carbs on the milk or not. I sometimes like about 3 coffees in the morning (at work) so I am going to monitor what the sugar levels do etc and pump if need be. :crazy:
Once every thing settled down after me low/high on Saturday, things have really stabalised, at 5.30pm Saturday night it was 5.6 and it has gone no higher than 6.5 (Sunday 6.30am) I monitored it though the night and early hours and the results were excellent 5.6-6.5.
I just hope I can keep the pattern going, for years I can go to bed at 10pm ish and sugar can be 5.6 wake up at 1-2am and sugar can be say 12+, so once again inject insulin :thumbdown:
I do hope Sophie has a good day today and also Monday at school, I shall be thinking of her on Monday.
I have got to do a cannular change this evening as my wife said try to leave the cannular change to about 6pm the on tuesday you can do it at the same time. I change every 2 days as I have a metal needle in the cannular.
Anyway my love to you both and hope Sophie is OK :D

Hi all
Been to the hospital today to see Accu check REP and DSN, the Accu Check Rep brought along a Accu Chek colleauge from Austrailer. They were all very pleased with my BG results and pump settings, I had a couple of highs and lows for the first few days of been on the pump, but the readings have been between 4 & 9.
So I am also pleased with everything as it stands, I am to see my DSN on 26th Jan 2012 with all the results fron today to 26th Jan 2012 to see if anything needs adjusting. I hope is going wel for Sophie and Riri :thumbup:

Nice to hear from you and so glad things are going well for you.
Sophie also had a check up yesterr her hbac1 in October was 11.9 and now after just 3 weeks on the pump has dropped to 9.2. The hospital are so impressed. Sophie has also put on 4kg in weight since oct, her weight has been a huge worry for a few years and if she puts weight on its usually just a few ounces. The pump has been an absolute god send and when I think back to just a month ago how apprehensive and unsure we both were as to whether we were doing the right thing it doesn't seem possible to be where we are now.
