Can you provide more context?
I am familiar with the DIAB1 form for your driving license but the dvla do not publish any other diabetes forms on the internet.

The diab3 form is sent directly to your consultant, they have requested I have a medical before renewing my license
How did this go? Was it a medical examination?
I am trying to find this form to find a private doctor to do this as my GP has a wait list of 8 wks!
