Diabetes and surgery (soon)

Hi, I'm an old diabetic, in that I mean I've had it a long time, over thirty years and have used insulin from the start. I inject four times a day, Humalog and Glargine for night-time. I am having surgery on Wednesday 23rd, to reverse an Illesostomy (stoma bag) and a parastomal hernia. Surgery for this was done as an emergency in Feb 2009 when my bowel perforated and I nearly lost my life. I have tried Metformin, but it makes me sick and because of this stomach surgery, is not good. I have dreadful blood sugars as a result of the stoma, ranging between 15 and 34. I can't find a balance and wonder if anyone else has had this problem. I am also waiting for treatment to shatter a very large kidney stone. My diabetic care is pretty bad where I live. I haven't seen a specialist Doctor in the three years I have lived here and the nurse is not a lot of help! Any information would be greatly absorbed! Many thanks.
