Insulin needs to come into the cabin with you. The temperature fluctuations in the hold might make the insulin ineffective.
 
Hi does insulin get packed with clothes etc or does it go in as cabin luggage?
All my D stuff comes with me in the cabin. I don’t loose sight. You don’t want it to freeze in the hold?( there is also one issue with depressurisation too.. flying at high altitude?)
You may find more air bubbles needing a larger “air shot” from being in the cabin? Just my experience.. Just don’t let it freeze..
 
It’s not just insulin which needs to stay with you in the cabin, it is all of your diabetes kit and any other medication. Not because of pressure or cold (this is not the case in the hold, animals are transported in the hold). As @Hotpepper20000 mentioned, your hold luggage may get mislaid. I do not want to be separated from the stuff that keeps me alive.
if you have a lot of medication (the advice is to carry at least twice as much as you think you will need in case your trip is extended or medication is lost or damaged), some airlines will allow a second carry on bag. It is recommended to contact them before you travel.
The other advice I would give is to keep your hypo treatment with you in your seat rather than the overhead locker. I experienced a hypo once whilst taking off and the seat belt sign was on. I had to chose the wrath of the air crew (and tuts from fellow passengers) over an extended hypo.
