Type 1 Freestyle libre 2

Hi guys, stupid question most probably. But I’m applying to have the libre 2 and have something to fill out before my “training session” I’ve filled everything else out except the part where it says Title…. Apparently I should find everything in the email I’ve received, does anyone know what title is please??

To my mind they usually want “Mr, Mrs, Dr or ms.” As a prefix?

I know what you mean. But all it has is title and that’s it, it says all the info is in email. Then says the time and date of the appointment and that’s it, I filled out my name and address on the previous page of the online form..

Sorry that was written quite badly, what I meant was that on the previous page was the usual name and address form, the current page I’m on to fill out says title, Date and time of appointment (that I have to choose myself)and that is it.

OK? I have to admit I’ve not had to fill out forms such a s this.
Could it be the title of the training course proposed by your endo or nurse?

My guess is that it's the course title, perhaps "Libre 2 Training Course," would suffice? Though if you've been offered a list of dates I'd have thought they would all be Libre training dates. Doesn't sound like a particularly well structured email you were sent.

You’re not wrong there, the link I’ve been sent to scan is apparently a new one as the old one wasn’t working, but when I try to enter my details it just says I’ve completed the form already, so Now I’m still none the wiser.
