gliclazide query

Hiya I've just had my diabetic review and I've gone up from 6.1% to 7.2.

The doc asked to see my last 7 days of test results and it seems i am spiking up to 14 2 hours after meals.
I currently take metformin 2g and prandin 2mg, to which shes now added gliclazide 40mg after evening meal, tho initially i was wary as had been on them before and had bad hypos.

Anyways have took first dose of gliclazide after evening meal and tested blood as requested 2 hours later and my bg was 12.4 so its not done anything.

Its been a few years since was last on gliclazide, so cant really remember how long it takes to work, is it straight away and if so should i go back docs in few days if still no improvement.

Hi. It is unusual to have both Prandin and Gliclazide together as they are both sulphonylureas that prod the pancreas to produce more insulin, although it's probably OK as long as the dose of both is taken into account. Gliclazide ought to work within a few days. It's probable that over the years since your last use of Gliclazide that your pancreas has fewer working islet cells to prod; I have the same problem. I wonder whether your doc has considered Sitagliptin (Januvia) a relatively new drug that helps with spikes without causing weight gain or hypos. I've been on it for 2 years now. It did help me initially but it has less effect recently. I'm afraid you may need to go onto insulin before long just like I may need to; sorry.
 
I found that gliclazide worked almost immediately. I take 40mg first thing in the morning and it dropped my levels by 1 - 1.5 mmol.

Can you see any reason why your level has gone up? Have you changed your diet or are now eating more carbs? If this isn't the case then it could be that your pancreas is getting more worn out. In that case gliclazide won't work as it stimulates the pancreas to make more insulin and if it is worn out then it won't make more. It is like trying to drive a car with no petrol in it.

i was taking prandin with my evening meal but it seemed to have stopped working, docs idea was to take prandin at dinnertime ( which i did n 2 hours after was over 11) and to take glic after my tea as well as take met SR twice a day.

my before tea bg was 7.9, only had small jacket with tons of cheese, 2 hours later 12. summit!!!

Now 5 hours later has gone down to 6.5 loll

I have to test for a week, before and 2 hours after breakfast and tea but think if no difference will go back docs on friday and maybe ask to increase the dose

However the nurse i saw yesterday did suggest i should go onto a 24 hour insulin but doc is dead against it :?:
 
Hi. My GP is also against me going onto insulin or at least was at my last review (quote: it's a last resort). The views of GPs and nurses vary wildly on whether to have insulin or not; if only there was more common ground on this. I'm using the NICE Diabetes Pathways doc as my reference.
 
Well i'm gonna try taking 2 x 40mg with me tea and see if that makes any difference, then if it does great and if not least i can go docs on friday and tell him they aint working :roll:

Funny though, two years ago glic gave me hypos and i swore never to try them again and now its done nowt so far!!
 
have been taking glicazide for 6 years about six months ago my sugars starting going up i could not understand it as I am very strict with my diet etc.
My Doctor increased my dosage that is really when my problems started it didn't control my sugars quite the opposite they went even higher i felt ill very dizzy my husband said i was erractic i had no memory and was losing weight .

It all came to a head when I passed out my husband took me to the hospital they did lots of tests but could not find a cause .My sugars were high i was refered to the diabetic clinic the nurse said diabeties was not causing my headaches and dizziness shen upped the tablets I was now on 4 80gr tablets .

My headaches were now violent pains i could only get round by clinging onto furniture the dizzines had increased .My Husband and my friends said I looked ill and were very concerned I was given insulin but before i started with it i felt strongly it wasn't the answer.

One of my friends had just started taking glicazide and it was working for her i felt impressed to find out which chemist she got them from .

My prescription was due and i got the tablets from the chemist my friend uses
i only took one my sugar level dropped from 16.1 to 5.6 i could hardly believe it within a week all my symptoms had gone and sugars were now normal levels

It was the previous brand of tablets not me .
 
Tried 2 x 40g and 2 hours later, bgs bout same as normal after btea ie 12.2

So should i go back doctors or keep trying for few more days???
