Type 1.5 Gliclazide

I have just seen endochronologist regarding my raised BS. He had thought I may have MODY as strong family diabetes on maternal and parternal, inc brother - both dx 10 years ago with T2, but neither of our parents diagnosed. He is now thinking I could have LADA due to sudden loss of BS control towards end of 2015.

He has prescribed a low dose of gliclazide - 40g - alongside my SR metformin of 2000g per day. Really worried about side effects and/or hypos. Never be happy at taking new meds.

Advice from the experienced please?

Also getting a cardiogram as slight heart murmur and kidney scan as I have some protein. Happy days . I am relieved at getting professional help.
 
If it is MODY then gliclazide is standard medication and is very effective in most MODY types. Hypos can be a bother - you may find you need to tweak timing and size of dosage to find what works best for you, but nothing to worry about. Have you adjusted exercise and diet? As said above, GAD and C-peptide tests really should be done if LADA is suspected -- it's worth asking about this.

I agree that this needs to be based on a GAD/C-peptide result. If it's MODY then your treatment will be entirely different than if you're 1.5/LADA.

My personal experiences with gliclazide have been poor. It worked for a minimal amount of time, but I think by the time we need extra insulin production as 1.5/LADA (which gliclazide triggers) it's really just time for insulin injections. Gliclazide caused massive rollercoasters in my levels as one time I'd be running ridiculously high while the other I kept getting hypos with minimal exercise. I was exhausted from trying to manage my diet while dealing with these random insulin bursts my pancreas put out.

I was taking gliclazide until December 2016, January 2nd I ditched it and started insulin full time. As you can see from my signature, my HbA1C went down massively within 6 months just from having proper control.

If you are indeed 1.5/LADA then I'd inquire about starting insulin instead. If your insulin output is so poor already that you need gliclazide, then you might as well start insulin.
 
I was diagnosed type LADA by gad and c-peptide. Took gliclazide because doc recommended it, and it did absolutely nothing. A confused doc re-tested my gad and c-peptide to find no pancreatic insulin production going on. Did the gliclazide finish it off, or was I just at that point anyway? There's no way to tell.

I agree with DaftThoughts, you might want to consider insulin instead.
 
Dr is an endocrinologist who is a diabetes specialist. I am having blood test. LADA is mentioned - no doubt I will find out more post testing. Yes, he talked about insulin - oh my goodness something I had not really considered I would not - really worried about hypos - plus I assume that means contacting DVLA. Husband has Parkinson's and also on 3 year only licence, but rarely drives. I have driven since I was old enough. We are only only mid 60s. But, I know my BS must be lowered and other options are really not cutting it.

My GP wanted to put me on gliclazide (in addition to metformin) when my HbA1C went up to 54 from 48. I wasn't happy as I reckon I'm 1.5/LADA (but no tests to prove it) and I thought gliclazide would only do my poor old pancreas even more damage. I suggested insulin instead. He referred me to the local hospital where the consultant suggested that going on insulin now to avoid having to go on insulin later was a bit daft. I didn't agree but when an HbAC1 reading taken at the hospital came back 64 (I don't believe it with hindsight) I agreed to gliclazide (80 microgrammes a day). HbAC1 fell back to 52 for a while but was up to 61 within 3 months and doubling the dose hasn't changed it. So my prediction that gliclazide would make things worse came true. I'm now trying to hold the line by reducing carbohydrate and upping exercise. By the way, I weighed about 65Kg before the diabetes, had Hb1AC of 100 when diagnosed and I now weigh 60 Kg.
 
I think I have misled readers as I referred to my GP. He is not a GP he is a Dr who works in a Diabetes Centre and local hospital - he had put me on gliclazide and requested I get more tests next week. He also is referring me for cardiogram due to slight heart murmur and kidney scan due to recent infections. He gave me through examination and did mention insulin - he suspects LADA as possible but obviously tests need to be done and I guess he said next week to see if gliclazide is working.

Update - on 40g gliclazide and my BS down to 6 - 7 as opposed to 12 - 18 and one day, after Atlantic night flight and undiagnosed kidney infection, it was 23! Seeing endo in Dec and DB nurse next week.
