Type 2 Going round in circles

my diabetes seems to be going round and round in circles and I can't seem to get off.
I am type 2 and take 2 x 1000g metformin extended release with my evening meal. 2 hours after dinner my reading was 11.1 and normally first thing in morning it is between 5 and 7. My ha1c (not sure of spelling) last week was 6
What happens is I wake up in night sweaty and desperate for toilet and then it takes about an hour to get back to sleep so when it comes to morning I am tired
Anyone else experience this my gp advice is to keep a record of when I get hot or feel unwell. I have just woken desperate for loo 2 hours after going to bed. Made some camomile tea so hopeless can get back to sleep
Had lots of blood tests all ok

Could you tell us what things you eat of an evening, and when.
Also assuming you're female, what's your age and menopause status ( if you don't mind)

I am 66 well past menopause
Breakfast same each day 2 slices of whole grain bread with thin spread of butter
Morning tea 2 whole grain crackers with cheese slice
Lunch wrap or sandwich with thin slice meat, tomato, carrot and onion. Fruit usually strawberry or blueberries or rockmelon depending on season
Dinner pork, chicken or beef generally just BBQ cooked with vegetables and potatoes if no potatoes rice and then diet jelly
Doesn't vary much just the type of meat.
On a Friday we have takeaway last night was half a beef kebab and chips and the reading 2 hours after was lowest dinner for week at 8.9
I eat the same brand of bread grain and oat (burgen)
Perhaps it's time to reassess everything I eat

@Jean1954 ... that, to be quite frank, is NOT a good diet. Rice, potatoes, bread and chips are a no go zone as they are high carb. Carbs are sugars after they're absorbed. More eggs, bacon, oily fish for starters.
 
When I look at what I eat it is a terrible diet. I have been to see a dietician and their recommendation was cut out all starchy food like bread, potatoes, pasta etc and eat things like lettuce, cucumber and green salAd vegetables not of which I have ever eaten. With my dinner I have at least 2 of the following cabbage, sprouts, carrots, pumpkin and 1 potato or small service rice. I need to cut out the bread/wrap at lunch and just have vegetables. Going to have a egg at breakfast with some toast. I have takeaway once a week with my grandchildren and try to have something healthy which sometimes includes a few chips. No other snacks are eaten by me.
Can someone recommend a good book or website on healthy eating as that's what I need to do

www.dietdoctor.com has some great low carb recipe and food ideas and is a great source of info about carbs and reducing consumption.
Green veg are fine anything grain based or starchy will likely be very bad. so bread, rice, potato, pasta obviously not good.
Eggs and bacon for breakfast (or coffee with double cream) .
Meat and salads for other meals.. cheese wrapped in ham as a sandwich but no bread.
There are lots of alternative things to eat which end up being far nicer and kinder to your blood sugars.

Edit to add your diet isn't really terrible but could do with some tweaking to make it more T2 friendly.
