Does a HB1AC of 48 require metformin, I’ve been good the last 3 months following keto, with a few cheat days I’m now going carnivore hopefully will push me into pre diabetic range but I’ve got to see the diabetic nurse and we always argue if I should be on medication or not there will be fireworks…..

48 is still higher than it should be and the risk of stopping metformin is it may increase slightly again

However you say you argue about wanting to stop.. you do realise they can’t force you to take any form of medication the choice is yours

So you could choose to stop and see if your diet controls it, but you risk a ‘I told you so’

But for the time being until your numbers are slightly lower you might be better off following the given advice

My personal opinion is that it doesn't require metformin, but it does require doing something about it. That could be metformin or other medication, or a carb-restricted diet, or whatever you find that lowers your blood glucose to normal levels.

If you have a look in the "Success Stories" section of this forum, you'll see a number of accounts from people who have brought their blood glucose levels down. You might want to discuss those methods with your DN.

@Laconic - are you currently on Metformin? is this a question about arranging to drop it, or are you expecting a discussion about starting to take it?

@Laconic looking at your past figures in your signature block it looks like you have done amazingly well in the past. No reason why you cannot get those blood sugars down, given that you are only very mildly diabetic at the moment. Check out those success stories @KennyA suggests.
