HbA1c test at GP's and other advice please

Dear Forum Members

I have been diabetic type 2 for many years. Recently my wife and I were told by our GP surgery that they may only be testing HbA1c once a year as opposed to twice yearly previously. Does anyone know if they are within guidelines to do this in the UK and do you have an opinion. I feel that because the test is only a snapshot of the previous three months I feel the test should be taken four times a year, but that's only my opinion. Once a year seems extreme.

Also, I was taken off my metformin due to my HbA1c being good. The last two figures have been:
May 2023 = 38.0 mmol
Sept 2022 = 36.0 mmol
Whilst I understand that these are good I struggle to understand this due to my figures being high virtually most days (I have used different meters over time). My fasting blood sugars (I have not eaten in the previous approx. 11 hours) are pretty much between 9 and 11 mmol. This, obviously, raises my figures for the rest of the day. In the past I have raised this with the diabetes nurse who says there is nothing to worry about and that most people’s glucose levels are raised in a morning. This might be the case but I see those figures as high and as I say, they do not help with rest of the day’s figures. Am I right?

I also have to now buy my own meter supplies as the surgery will no longer supply these to me. I understand this is a common situation. Is there anything that can be done to change this?

I agree 3 monthly HbA1c tests would be the most helpful, however NICE Guidelines say every six months once stable, see here section 1.6:


But yes these same guidelines don’t recommend self testing. I personally do test as a lot could change in 3, 6 or 12 months, some say not testing is like driving without a speedo in your car, you only know what’s happened when the speeding ticket arrives in the post.
