Hello from a LADA type

Hi!

I've been reading up on many topics on these forums and am both astounded by some of the bizarre stories, and comforted in knowing I'm not alone.

I'm a 28 year old Dutch woman, diagnosed initially as a type 2 diabetic in 2014 due to obesity. I'm 186cm/6'1" tall and consistently weigh between 120-124kg / 265-273lbs. When I saw my diabetes specialist she insisted on having me tested for LADA because of my age and the history of type 1 and type 2 diabetes on both sides of the family. The test came back positive and I got started on metformin without insulin. After some time struggling with wonky high levels, glicladize was added to my regime. I was doing quite well for a year, but beginning of 2016 my levels were out of whack again and I couldn't reign them back in.

I ended up with a fasting value of 8.6 in the morning, biked to my doctor's office (15 mins) and then back home, and had gone up to 9.8. Not cool!

Mid 2016 I got sick and tired of the extreme fluctuations in my bg levels. My A1C wouldn't budge and I was exhausted all the time. We agreed to start insulin and keep the oral meds and I'm now on 10 units of Toujeo basal insulin (no bolus yet) as of July 22nd. My fasting values have become absolutely perfect and I'm noticing massive improvements during exercise (I'm no longer panting like a dog on a simple bike ride and I no longer have muscle fatigue within 5 minutes), although I do sometimes still spike above 10 after meals. My problem is mainly that my own insulin production is all over the place, which sometimes leads to random hypos as well as hypers depending on how active I am.

I'm discussing my day curves with my doctor tomorrow. My overall averages are pretty good, I'm just worried about the spikes after meals. Hoping to discuss these with her and see if bolusing is a bad idea in my current condition or something I can do to correct spikes more quickly instead of waiting them out.

In the meantime I'd love to connect with other LADAs, especially those who struggle with being overweight and can't lose it. I exercise 5 times a week on average because biking in my only mode of transport (and I live in the not-flat parts of the country in the South, no bike ride goes by without a minimum of 3 steeps hills to climb!), I eat no more than 2000-2200 calories a day (which is rather low for my height and activity level), I try to stick to 40g carbs or less per meal etc. but I'm not losing a pound or an inch.

Hi and welcome. I think you may need to reduce the total carbs per day to 100gm or less unless a high level of exercise demands it. Think carbs not calories, but make sure using the meter that any exercise doesn't take you too low. Also have enough proteins and fat. I would certainly consider adding a bolus if you have spikes. My DN added a Bolus after 1 month on just Basal due to spikes. You don't need to use the Bolus for every meal and with carb-counting you will know how much to take.

Have you found the LADA section of the forum? Lots of discussions there on managing your particular brand of D, which I hope you find useful.

The spikes may be easier to manage if they remove the oral drugs and add a bolus. They may however be waiting for your honeymoon to end.
I don't know about the weight issue, but Diabell made a good suggestion - do you know how many carbs you are consuming? You'll need to start thinking about carb counting if you are going to end up on bolus insulin do it might be worth looking at.

I gain weight with anything over 20 carbs a DAY. My carbs come from above ground veggies, avocado and some nuts and seeds.

As far as your insulin production and your bs all over the place, could it be the glicladize causing the hypos?

Welcome! I think you should introduce bolus into your insulin therapy.
 
Hi everyone, thanks for your responses!

My doctor decided we need to try and manage the spikes with an extra dose of gliclazide for meals on days that I'm running high and do without when I'm fine or low. I'm a little bit skeptical but willing to give it a shot. I was doing very well on the oral meds before which is probably why she feels it's a better option. I'll be reporting back to her in a week or two and then we'll see from there! Thanks for the suggestions.

I want to low carb but currently can't do this safely. I think I'll head over to the LADA forum and discuss things further there though. :)
