Hi I am all for surveys and often conduct them informally myself but I do feel when posting a survey you should say who "we" are - users should know whether they are being asked by an organised group, medical pratcitioners or just interested individuals. Secondly with talk of "medicare" I would imagine this is not primarily aimed at UK residents where the second question would be about National Health Service care.
