High after changes made

Hi, hoping I can get some help on here please. Last year I'd been having more lows than was acceptable, got hold of my pump team, they made adjustments. The nights have definitely improved but honestly the days are terrible at the moment, really bad highs, example today, started off fine, had breakfast, walked the dogs - numbers were fine, had bagel & bacon for lunch, gave correct insulin & extra for sauce, numbers have crept up ever since, despite correcting now sitting at 22.1 nearly 2 hours later!

I spoke with my DSN about a month ago as I was feeling a bit frustrated with the highs I was getting, she didn't seem bothered but I need this sorting as my TIR is awful


I have been using Medtronic pump for over 17 years, currently using Medtronic 780g with smart guard
 
I’m sitting at 21.9 with 1.474U on board & pump offering no correction?!!
 
Hi Sasha. It does sound like your carb ratios need adjusting.

As a quick help to try and lower your present level have you clicked the bottom arrow key when you’re on the Home Screen? It would be interesting to know if it suggests a Bolus adjustment which you could deliver.
 
The pump will only correct to a certain extent before it won’t correct anymore without you interjecting and doing it yourself, as far as I remember it’ll give a percentage of your TDD as corrections but once it’s maxed out it won’t give anymore unless you either press the down arrow to manually give it or enter a blood sugar and manually give it.

There’s only 2 things you can change in smartguard to help with overall blood sugars, your AIT (most people set there’s to 2 hours as that’s what Medtronic recommends although it doesn’t work for everyone) and your carb ratios. It seems more than likely if you’re high 2 hours after your meal your carb ratios aren’t quite right and I would look into speaking to your doctor/nurse about that.

Thank you, I have been waiting for a reply from my nurse since 1st February
I emailed again last night so hoping I may get a reply sometime this week

They do not like my making changes without consulting them first - so frustrating when I try to manage my diabetes as best I can & want good control - but get told well at least you’re not having so many lows
I mentioned my highs (9-12, obviously sometimes higher) & get asked what’s the problem??

It did finally offer me a correction

Thought I would just continue from this thread rather than start a new one
So saw the consultant last week, mentioned to him about the afternoon highs - he made some alterations to my pump
Today has been a nightmare
I am attaching a screenshot
So went to the gym about 11.45am for a lower body session, my numbers just kept going higher & when I went into the pump it offered me corrections - which I did
Came back for lunch - gave the right carbs count for my lunch & a brew- pre bloused - had a bit of puttering about to do after lunch so was moving - about an hour after lunch - numbers were out of range again!! Pump had done corrections & I was still able to do them to- when I went into the pump
I just don’t understand what is going on
It’s like now I’m sitting at 15.3 with 2.7 active insulin on board
I am really struggling with this

Any advice please?
