Type 2 High blood sugar

Im on metformin 2 x 1000 , januvia, and ramapril for high blood sugar. HbAc1 has started creeping up, now on 69. Doctor has now described canagliflozin, just wondered whos tried it and if it helps. I no stress is a big factor in my diabetic life., and-am really trying to control this with diet and excercise. Dont feel im doing anything different for my sugars to be rising.( apart from stress).
 
Can you tell us what you eat and drink in a typical day?

Stress certainly can play havoc with our blood sugars.
 
Hello. Have you been diagnosed for very long? And did your gp have any idea why this should be happening now?
 
The canagliflozin is a good drug; good is a judgemental term and not helpful. It reduces the HbA1c and on its own, does not cause hypos. It also reduces your chance of heart attack and stroke, and preserves your kidney function - useful. It drops weight a few Kg and improves BP a bit. No other tablet does all this.
You have a good GP who is going for the best tablets when he/she could have added a sulphonylurea with none of these benefits or pioglitazone with some of these benefits for 1/10 of the cost.

Aw thanks, yes i no my mistake, Ramapril 4 blood pressure, i am a nurse lol. I have porridge with water, 1 coffee, ryvitas , boiled eggs, 4 lunch with marmite, meats, for tea with veg or salad. Ive been diabetic for 2 years. I started at 20 stone , got down to 17, and now 18. Im active, i walk, work and have my grandaughter who is 3 most of the time., who lives with me.my bmi i think is 43. I feel i eat well not many carbs, and not many sweets.

I know OP has replied that the ramipril was prescribed for blood pressure but small doses of ramipril can be prescribed to diabetics without bp issues for kidney protection especially if there is slight damage - just thought it was worth saying:)

Yes, the ramipril can be given to protect one's blood vessels. The HOPE and Micro-HOPE studies looked at diabetic people with other risk factors for stroke and heart attack including micro-albuminuria (small amount of protein in the urine). They found that 10 mg ramipril at bedtime avoided heart attack, stroke, death - useful. The makers tried to say that it was something magic about the ramipril cos BP next morning was unchanged - but in effect, the BP went down a lot during the night, so a lot of the benefit was probably due to blood pressure reduction. Ramipril 2.5 mg at night did not have any effect, which many doctors seem to have not noticed.

ARBs such as irbesartan and losartan reduced progression of renal disease, but in a diabetic person with T2DM, for the individual, avoiding strokes, heart attack and early death is probably the most important thing.

ACE inhibitors are known to prevent renal impairment, dialysis and death in type 1 diabetes, and I think there was an enalapril in Type 2 diabetes trial which showed avodiance of renal impairment in T2DM, but no one reports this study cos of the dubious ethics of the investigator.
