Hi guys.swapped from lantus to tresiba few months back due 2 nightime hypos.Since i made the swap I am finding out (thanks to libre) that my sugars are constantly high throughout the night but start to come down around 6am.

Tried increasing my dose considerably but no difference.

When do you take your dose? I know they say it doesn't matter what time you take it at but I've found taking it at night results in much better nighttime blood sugars than if I take it in the morning. I've also found splitting the dose works even though you shouldn't have to.

I have the same trouble since I changed from Lantus to Tresiba , mine happen around the 5/6am mark and I take mine quite late at night around half 9/10 pm. I also find my hypos have changed to not real warnings like before it’s straight into it other than a slight dizziness or headache but when ur asleep that’s not always easy to detect. X

Hey @scotteric, I've just changed over from Levemir to Tresiba, and I'm trying to stay patient with it. First of all, I was dropping from a high of around 15 at 3am, to around 5/6 in the morning, then good all day. Until around 10.30pm when I would start to rise up every single night. The drop was way too much so obviously not the right time/ratios etc.

Changed my dose to the morning and I get a flat line over the course of the night, just need to bring those before bed numbers down. I'm losing a bit of my motivation as I am running so high all the time. I think its because I need a lot more overnight?

How did you go about splitting your Tresiba? I can see on the Libre that I am going high every night once my novorapid has worn off so I obviously need more at that time.

I’ve just changed from a split dose Lantus (10 units morning and night) to a morning dose of Tresiba (18 units). Had my second dose this morning. My doc said the units were pretty much equivalent, but to take 18 instead of 20 to start with, and then increase if it’s a bit high. My levels were a constant 9-10 all through the night, so I upped it to 20 this morning. Probably too quick, but I do tend to have a “bull in a china shop” approach to things... Go big or go home, right? :D

I dont know... the whole point of tresiba is 1 dose that drips out like regular insulin over 24 hours or so ... it took me 4 weeks to get me steady night and day but a tweak on the 4th day and then the 10th etc eventually it all calmed down and steadied off at 7. Personally ...def give Tresiba 4 days to bed in..I know that's not good if youre running at 14 but you have to go through the process to get the results and I believe ....completely worth it ..buggering about with split doses and that stuff is olden days ..take Tresiba as they recommend just be patient. Sorry if I talk out of turn but I love it and I was setting alarms every hour in the night for 8 month on levimir afraid of nocturnal hypos... i set one now at330am .dont all t1 ?

I recently changed from Lantus to Levemir. I went from 10+ during the night to hypo just about every morning for a very long time after taking 20 units of Lantus at around 10pm, to taking 5 units of Levemir at 10pm and 15 in the morning and my problems have largely gone away. I also get dawn phenom that I counter with Novorapid before going to bed, and being a night owl it isn't that difficult to estimate how many units I need.

Honestly I really couldn't find a method that worked consistently for me and I went back on my pump. MDI was an experiment for me, as I had been pumping for 11 years and started only 3 months after diagnosis. I had been having constant site problems and figured I'd never given MDI a real chance, so wanted to try it for a while. I could not find a dose of Tresiba that worked well for me, I was always too low in the morning hours and too high at night. Splitting it started to result in morning and night peaks where I dropped extremely low. I suppose I could've kept trying things but it got to a point where it just wasn't worth the effort. I certainly don't get perfect results on the pump, but at least I can make adjustments on the fly and raise or lower my basal dosage for periods of the day where I need more or less, both of which are impossible to do with Tresiba, Levemir or Lantus. Good luck to you and I hope you get it working! I'm convinced after my experiment that a pump is the way to go (at least for me) until they develop a smart insulin that can adjust based on the body's needs. I'd strongly push for one if you're able to and haven't tried one yet.

I had been steady 70% in target zone but over the last 3 weeks I have had a change. I go a bit low 4-5 before bedtime say 11pm and go to bed and then find I've gone high through night 15-16 which lasts until mid morning. I even tried 5 units of fiasp on waking and nothing much happened for 2 hours. I've been raing mt Tresiba dose steadily over thlast month from 25 to 32 now. Any body any ideas?
