Why is it that when I eat sausages and I calculated the carbs my blood sugars start off very low and then rise slowly over the four hours to a high reading ? Is it best to take some of the Novorapid and then give myself the rest some hours later to bring down the high readings over six hours instead of four.

Possibly the protein has some involvement in the delayed rise, as well as the high fat. If you know how big of correction dose you have to take you can easily plan for it. It does sound however like your initial dose is a bit off, try giving yourself a slightly higher dose. If you rise slowly from start you can wait a bit with injecting as well.

High-fat meals can cause a delay in the absorption of the carbs which can lead to postprandial hypo's if the full dose is given upfront, plus high-fat can cause insulin resistance for several hours after eating and this is why you may find you need more insulin to bring your bg levels down.

On MDI I would split-dose and sometimes even needed a third injection when eating high-fat meals, it's really trial & error to find out which way works best for you.

Have a read of the following which explains things better than I have:

Hi . . .

I've seen it mentioned a lot on this forum, to split-dose Bolus Insulin when eating low-carb and/or high-fat meals. An alternative solution might be to use a different Bolus Insulin.

I am now on a Ketogenic (LCHF) diet, having stumbled along on a standard low-carb diet for many years before that. I found that my Bolus Insulin (Humalog) really didn't work, for the style of food I was eating. The profile was too steep, compared to the effects of the food.

When I discussed this with my Doctor, she made a suggestion to switch to Huminsulin Normal (it's called Humulin S in the UK) which has a much flatter profile. I'm very thankful to my Doctor for this suggestion! The profile of this Insulin seems to match beautifully with the diet that I'm on, and I can stick to LCHF meals using only 1 Bolus injection at mealtime. My levels stay very flat in the hours after meals.

I've moved location a couple of times since then, and registered with different Practices. So far I've never had any problem getting 3 Insulins on prescription (1 Basal; 2 Bolus). I use Humulin S for my standard LCHF meals, and Humalog for my Carb binge days and any correction doses.

There are lots of Insulins available, with a variety of profiles. If anybody has a consistent problem with matching Insulin to food, I'd definitely recommend speaking with a Doctor, and/or researching the different Insulin profiles. There's probably an Insulin which really matches what you need.

Regards :)

Where can I find info on the diet you mentioned as I may give it a go and see what transpires. Anything new I'm up for.

Hi Ms 60 . . .

This is my favourite topic, and therefore I'll try not to bulldoze you with info!

If you search on Amazon for "Ketogenic" or "Ketosis", you'll now find loads and loads of books. Seems like most of them are recipe books, or at least devote a chunk of the book for menu/recipe examples. Can't speak for most of these books, as I've not read them.

I've read mostly the books explaining the concept and effects of the diet; this is what most interested me. I think my favourite amongst these was "Keto-Adapted" by Maria Emmerich. Explained the details in a down-to-earth way, without being too preachy. And it's not too long or complicated.

I bought a copy of "The Ketogenic Diet" by Jennifer Williams for my Mum, after she became interested by my diet and how I was managing things. One year later, she was the thinnest she had been for 30 years, and feeling pretty darned good. (And she still is). She's not diabetic and can be much more liberal with the diet than I can, but I think she got a lot out of reading the book. It's a fairly basic overview of the diet, and doesn't go into too much detail.

There's a good online resource here:

Hopefully this is helpful . . .

I have loads more info! Happy to give you any more info if you send me a Private Message.

Regards :)
