Hypo Actions/ Prevention

Now that my BS is in much better control, HbA1c result last week of 45, in evenings post supper my BS is regularly in the area of 4.8.
I don't feel at this level that I'm going into a Hypo but I think I could be near to this condition.
Before my BS was controlled better I used to get advanced warnings of Hypo's, going sweaty/confused etc, but now I don't seem to get these warnings.
So question is at what BS level after Supper should I take preventative action to stop Hypo's when I'm sleeping?

Johnoo - It would be really most helpful to understand which variant of diabetes you have, and what medication, if any, you take.

1 Type 2
2 Medication:- Morning 1 Gliclazide, 2 Metformin, 1 Alogliptin.
Evening:- 1 Metformin
Previous to good control I was taking in the evening 2 Metformin & 1 Gliclazide, but this has been reduced in agreement with the medics

Because the glicizide is a blood glucose lowering drug, you should speak to your GP about adjusting your dose because you are currently in or near levels.
The more you keep doing low carb the need for meds should reduce.

OK, thanks but I take the Gliclazide before Breakfast. My Fasting levels, average, is BS 7.7 & 7.6 2 hours after Breakfast.
It is the evening where I'm tending to go low.

I would say so, yes.

Also, you test before bed? That might give a good indication of where your blood glucose is sitting later in the day.

There is a general idea type 2s don't have to worry about hypos, unless they have other medical conditions which may drive blood glucose down, or they are on blood glucose reducing meds such as Gliclazide. Metformin is not considered to cause hypos, although people can still experience hypos when on Metformin. :)

So, if I were you, I would have a read up on how Gliclazide works, and how long its effects continue after taking the tablet.
By that, I mean that if Glic ceases to work long before your evening meal, then you won't have to worry about a hypo after that meal. Hope that made sense :)

This link suggests that the lasting effectiveness of Glic depends on whether you have the slow or quick release version.
But I would suggest you check the brand name with your pharmacist, or your doc, for confirmation of that!

For me I'm on gliclizide 40g morning and evening and they are perfectly normal numbers for me - a hypo isn't until you go below 4 so if you feel fine at those numbers I wouldn't worry about it - if you feel wobbly try to ride it out and don't over treat - if I hit below 4 I have 2 glucose tabs then test in 15 mins usually brings me back up to the high 4s

If your HbA1C is 45, which is pre-diabetic then the need to take Gliclazide is minimal and possibly dangerous if it is giving you hypos by dropping your blood sugars too low.
 
Thanks to all I'm just going to monitor if I go under 4.0 & then adjust with care.

I'm satified that my Medication is correct for the HbA1c levels I'm achieving
When I get my next review these will be reviewed as standard practice.
