Insulin in the blood

Hi all. My fasting insulin level is 17.5 MU/L and my fasting glucose level is 7.0 mmol/L. I'm taking insulin of 34 units, 1000mg metformin and 100mg vildagliptin day and night.
Should I increase my medication since my IR is high ?? TQ
